Job Description The Clinical Registered Nurse (RN) II is responsible for managing individualized patient care by promoting and restoring patients' health in accordance with medical and nursing plans of care and established policies and procedures. The Clinical RN II collaborates with physicians and multidisciplinary team members in the development, execution, and evaluation of the multidisciplinary plan of care. The RN assumes a full patient assignment and can plan, analyze, intervene and set goals for patient care accordingly. This nurse provides nursing services to patients and families in accordance with the scope of the RN as defined by the Virginia Board of Nursing. Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family. Recognizes potential problems and responds to general information/data indicating urgent and/or emergent risk to the patient. Initiates emergency resuscitative measures as appropriate. Assumes responsibility and accountability for effectively managing the care of a specialty patient population. Provides care in a non-judgmental, non-discriminatory manner that is culturally sensitive to the patient and family, preserving their autonomy, dignity, and rights. Collaborates with interdisciplinary team members to ensure patient care interventions are appropriate. Evaluation Evaluates patient’s responses to care based on effectiveness of nursing interventions/actions in relationship to established goals. Initiates changes in the interdisciplinary care plan based on this evaluation. Problem solves with the specialty team to improve patient outcomes. Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution. Patient Education Identifies learning needs of patients/families and teaches by adapting standard information based on needs. Ensures that the patient/family demonstrates knowledge, within the abilities of the patient. Communicates with patients and family to identify their needs and include them in the plan of care. Acknowledges and supports determinations by the patient and/or family concerning treatment. Self and Peer Learning Assumes responsibility for assessing and meeting own learning and professional development needs. Maintains current competency in nursing specialty and requirements for department; attends required educational programs and may attend additional educational workshops; reviews nursing professional publications; establishes personal networks; may participate in professional societies. Seeks additional educational opportunities for clinical development and is either ready to learn the charge nurse role or currently functioning in a charge nurse role. Collaboration Makes sound judgments in the decisions involved in coordinating multiple, increasingly complex patient care demands. Supervises delegated care based on abilities of personnel and patient acuity. Reports changes in patient condition to appropriate personnel. Service Recovery Participates in service recovery and uses appropriate resources during complex situations. Handles conflict resolution in an appropriate, timely and consistent manner. Able to serve as a preceptor/mentor and works directly with employees to fulfill planned orientation to the department. May assist in the orientation, instruction, direction, and evaluation of students assigned to the unit. Able to be trained for a charge nurse role or is currently functioning in a charge nurse role. Key Responsibilities Provide direct nursing care in accordance with individualized care plans, physician orders, professional nursing standards, and facility policies Monitor and assess residents within the LPN scope of practice and promptly identify changes in physical, cognitive, behavioral, or psychosocial condition Communicate significant changes in resident condition to the appropriate RN, physician, nursing leadership, and responsible party Safely prepare and administer prescribed medications, treatments, and therapeutic interventions within scope of practice Monitor resident responses to medications and treatments and report concerns or adverse reactions appropriately Coordinate with physicians, RNs, CNAs, pharmacy, therapy, dietary, social services, and other interdisciplinary team members to promote continuity of care Assist with admissions, readmissions, transfers, and discharges as assigned Provide residents and families with appropriate education regarding medications, treatments, care plans, and health management Supervise, delegate, and provide direction to CNAs and other nursing support personnel within applicable scope-of-practice requirements Ensure assigned resident care is completed safely, timely, and according to individualized care plans Maintain accurate, complete, and timely clinical documentation within the electronic medical record (EMR) Follow up on physician orders, laboratory results, treatment recommendations, and other clinical information as assigned Participate in infection prevention, resident safety, quality assurance, and performance improvement initiatives Respond promptly and appropriately to urgent resident needs and emergency situations Protect resident rights, dignity, privacy, confidentiality, and individual preferences Maintain compliance with facility policies and applicable federal and New Mexico requirements governing skilled nursing care Qualifications Graduate of an accredited practical nursing program Current, active Licensed Practical Nurse (LPN) license in good standing in the State of New Mexico Current CPR or Basic Life Support (BLS) certification Skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred Strong clinical observation, critical-thinking, and decision-making abilities within the LPN scope of practice Ability to recognize changes in resident condition and appropriately escalate clinical concerns Ability to effectively supervise, delegate, and support nursing assistants Excellent communication and interpersonal skills Strong organizational and time-management abilities Ability to prioritize multiple resident needs in a fast-paced clinical environment Ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members Commitment to compassionate, resident-centered care and positive clinical outcomes Benefits $10,000 Sign-On Bonus for eligible Full-Time LPNs Competitive pay Health, dental, and vision insurance Paid Time Off (PTO) Care Cash Program Referral Bonus Program Professional development and advancement opportunities Supportive and collaborative work environment Grow With Us At Cedar Ridge Inn, we are committed to developing and supporting our nursing team.
