FLSA Status Non-Exempt Job Role Summary The Team Captain RN and Triage Nurse RN are positions that allow for a collaborative approach to the patient's healthcare in a multitude of ways. Team Captain RN - This role is for the nurse responsible for the overall care of patients assigned to their team's providers. This includes risk assessment and coordination of care for high risk patients, tracking quality metrics, delivering elements of patient education, and serving as the principal point of contact for patients and other care providers outside of face-to-face appointments. The Team Captain supervises and directs the activities of team nurses and support staff. Triage Nurse RN - Triage is an interactive process between the nurse and patient that occurs over the telephone or face-to-face and involves identifying the nature and urgency of client healthcare needs and determining the appropriate disposition. The role of the triage nurse is to assess the patient, provide patient education, and crisis intervention. The goal of telephone triage services is to direct the caller to the appropriate level of care or service in a safe and timely manner in combination with providing self-care advice and direction. The Newly Licensed Registered Nurse (NLRN) utilizes a holistic systems approach to protect and promote client wellness and safety in the physiological, psychological, social cultural, developmental, and spiritual domains. Nursing interventions are interdisciplinary, age specific and are directed toward optimal wellness. Education, License & Cert: The NLRN must be a graduate of a Diploma, Associate Degree or Bachelor of Science in Nursing (BSN) program and have passed the state board examination by the New York State Board of Nursing before beginning employment. NLRNs who are graduates of a Diploma or Associate Degree program or possess a non‐Nursing Bachelor’s Degree must demonstrate completion of a BSN program within 5 years of hire. The NLRN must successfully complete a medication and IV challenge examination, as outlined by the hospital within the orientation period, as well as maintain CPR certification. Essential Functions Conducts individualized wound/ostomy specific patient assessments based on consultations received from hospital staff. Reassessment: ▪ Performs reassessment based on team standards, CPG’s/ND’s, protocols and changes in patient condition. Develops wound/ostomy educational materials for health care team, patient and family: Develop and revise clinical pathways for wound care. Revise teaching sheets, policies, and CPG’s related to skin integrity, wounds, wound vacs and ostomies. Provide educational materials for patients and staff in regards to common skin, wound and ostomy problems. Provide educational offerings/presentations for staff regarding skin, wound, and ostomy care. Revise the wound and ostomy formularies to assure the organization is providing excellence in wound and ostomy care in the most cost effective manner Provides leadership in the monitoring of prevalence and incidence of pressure ulcers within the organization. Develops, enters data, and compiles reports from skin, wound, ostomy data base. Promotes pediatric research by implementing research findings into practice.
