Reporting directly to a Nurse Manager, this position will supervise Medical Assistants (MAs) and, with other interdisciplinary leaders, manage integrated health services in Recuperative Care. Responsibilities include support of direct reports, quality improvement efforts for clinical access, and participating in an integrated team for patients discharging from a hospital who are too frail to heal on the streets or in a shelter. This will be done in accordance with current federal, state, and local standards, guidelines, and regulations governing CCH to ensure the highest quality of care is maintained at all times. This position is approximately 50% administrative and 50% skilled medical assistant care to the patients. The primary work location is the Stout Street Recuperative Care Center, but may include Stout Street Health Center, home visits, community-based outreach, or other CCH integrated care sites as needed for collaboration needs.
