Day Center CNA

The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Other duties as assigned. REQUIRED Active and valid state-issued Certified Nursing Assistant (CNA) license Current CPR certification or the ability to obtain CPR certification. Position Summary Performs general patient care to patients under the direction and supervision of the Licensed Practical Nurse or Registered Nurse. Responsibilities Assists patient with hygiene, mobility, hydration, elimination and nutritional needs. Provides routine treatments and maintains a safe, clean, neat, and organized patient environment. Maintains safety and security of patients, families, self, and other members of the healthcare team. Assures patient privacy, rights, and confidentiality. Follows standard precautions, always washes hands between patients. Assists with admission, transfer, and discharge of patients. Takes and records temperature, blood pressure, pulse, respiratory rates, food and fluid intake and output as directed. May have limited access to areas storing medications. Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards. Maintains compliance with all Orlando Health policies and procedures. Enthusiastically supports and participates in planned changes. Participates in nursing Performance Improvement activities as assigned. Demonstrates awareness of legal issues, patient rights and compliance with the standards of regulatory and accrediting agencies. Consistently utilizes time and resources effectively and efficiently.
