Maintains the Environment of Care, providing Homemaker Services in order to maintain the patient’s comfort and to provide a healthy, safe environment. The Hospice Nursing Assistant will have primary assignment to Home Care or Inpatient Care, but is expected to help in areas of need within any division/department of HOSG as directed by the Nurse Manager/RN. Assists with medication and supply pick up and delivery as needed. As agency need requires, may assist with clerical duties such as answering phones and/or call lights. Other responsibilities include maintaining inventory and organization of basic medical supplies and DME. KNOWLEDGE, SKILLS & ABILITIES 1. Preferred high school graduate or GED equivalent 2. Required completion of approved Nursing Assistant Certification training program with evidence of current State of Georgia certification 3. Minimum two (2) years varied work experience as a Certified Nursing Assistant preferred 4. Prefer training or experience with general office skills and medical terminology 5. Demonstrated understanding of and adherence to the hospice philosophy and ability to work as member of interdisciplinary team in meeting the unique needs of the terminally ill. 6. Must possess strong skills in core areas of Certified Nursing Assistant Scope of Services, as defined by State of Georgia. 7. Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively 8. Must be self motivated with strong organizational skills and demonstrated ability to prioritize tasks, handle pressure, and utilize critical thinking/problem-solving techniques. 10. Ability to operate clinical equipment as well as office equipment such as computers, telecommunications devices, etc. 12. Must have current Georgia driver’s license with satisfactory driving record and the availability of a reliable, licensed and insured vehicle for use on the job. 13. WORKING CONDITIONS - ADA INFORMATION Work environment may include hospice office, patient homes, nursing homes/ALF, and/or inpatient hospice unit with primary assignment to either Home Care or Inpatient Care environment. Expectation is to assist in either area as directed by RN/Nurse Manager. Generally assigned to a consistent shift with regular working hours according to assigned department. May require long, irregular, or flexible work hours including some weekend, evening, and/or holiday hours on either a scheduled or rotating basis. Daily interaction with persons under stress, having complex issues/concerns (end of life care), and/or with whom there is a need to respond to questions, problems or gain cooperation. Frequent/daily travel within service area required in Home Care setting. May also require occasional driving to hospital and/or pharmacy with either primary assignment in Home Care or Inpatient Settings. Physical work may require sitting, standing, bending, and frequent lifting or pushing objects (excess of 50 pounds), using correct techniques. May require extensive standing and walking. Likely to be exposed to blood, body tissue and fluids as well as other hazardous waste materials on a daily basis. Certified Nursing Assistant Phlebotomist

JOB PURPOSE: Provides each of the assigned patients with routine daily nursing care and services in accordance with the patient’s assessment and care plan, and as directed by the nurse supervisor. Under the direction of the manager, performs daily phlebotomy activities accurately, professionally and in a timely manner. KEY RESPONSIBILITIES: 1. Performs all duties and responsibilities according to the Certified Nursing Assistant (CNA) job code 0101, Department 6300 for HCC. 2. Handles laboratory data collection systems and equipment to ensure requests for laboratory tests are properly ordered, cancelled, and charged. 3. Correctly identifies all patients to avoid mistakes that can affect treatment. 4. Performs blood collections from patients, mostly older age group 5. Performs specimen processing, preparation and distribution of specimens 6. Maintains a constant awareness of policies, and procedures. Always ensuring enforcement of infection control processes when working with or around patients. 7. Effectively communicate with patients, reassuring them, making them feel at ease when you are present. Effectively communicates on the phone and face to face by being courteous, prompt and polite. Ability to document all information correctly 8. Orders and maintains laboratory supplies 9. Responsible for keeping all work areas clean and properly stocked. 10. Handles information with discretion and always maintains confidentiality. 11. Coordinates and consults with lab manager regarding lab related issues. 12. Able to work independently and in a team environment. 13. Comply with all OSHA, CLIA, and federal regulations. 14. Maintain a professional appearance and attitude. 15. Other duties as assigned. Patient Care Technician (CNA)

1. Contributes to patient database by collecting and documenting patient information, such as height, weight, allergies, vital signs, and intake /output measurements. Prepares patient rooms (beds and equipment) for receiving patient admissions, returning surgical patients, transfers, or discharges. Assists in and accompanies patients in the admission, transfer, and discharge procedures. Always correctly identifies patient by name for any direct care procedures and treatments or diet; checks patient’s name band for correct identification: assures proper name and that the ID band is secure on the patient’s wrist. 2. Provides general patient care, such as morning care, which may include bed bath, oral hygiene, combing hair, dressing patient, evening care, back rubs, changing bed linens, cleaning overbed table and bedside stand, straightening room, and other general care as necessary throughout the shift. Provides for patient privacy, at all times; closes room door and cubicle curtains for all procedures. 3. Prepares patient for meals, and assists with meals or feeding the patient. Distributes drinking water, ice, and other nourishments to patients as ordered. 4. Collects and transports specimens as ordered or requested. 5. Reports any changes observed in condition or behavior of patients in a timely manner. 6. Responds promptly to patient needs and call lights (whether that of assigned patient or not), and checks on patients regularly between tasks. 7. Provides for patient safety through preventative measures (bed controls, nurse/patient communication system, side rails, and patient education). 8. Demonstrates the following skills, competently, effectively, and safely as delegated/directed by a licensed nurse: Discontinue peripheral IV, discontinue indwelling urinary (foley) catheter, phlebotomy, EKG, bladder scanning, wound care- dressings (foam borders and preventative care); obtaining blood products from the Lab; nasopharyngeal specimen swabs; and ostomy appliance change. With successful completion of appropriate training and competency evaluation, may perform the duties of a Monitor Technician.
