Obtain and documents patient’s vital signs appropriately in the patient’s chart as requested by the bedside RN. 9. Demonstrates ability to communicate to the RN any observed changes in patient status and responses. Maintains awareness of the needs of the pediatric, geriatric and general patient population. 10. Demonstrates ability to handle patients safely and utilizes proper body mechanics and techniques; uses gait belt with transfer of fall risk patients. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed.
