The Eligibility RN is part of the In-Home Services team and partners with the local Enrollment team to determine if prospective participants meet the guidelines for enrolling in the PACE program. Schedules and conducts home visits, to make level of care assessments to appropriately determine participant eligibility. Completes respective documentation, in close collaboration with the InnovAge Primary Care Physician, and compiling supporting documentation for enrollment. Participates and assists with outreach programs. Serve as member of the Interdisciplinary Team (IDT), and coordinate appointments for prospects that require further assessments by other IDT members. Act as a consultant to external health care personnel about clinical apheresis. Assess patient status relative to therapeutic apheresis, prior to and during procedures, utilizing the nursing process, laboratory testing results, and other clinically significant procedures. Coordinate closely with physicians and nurses at a variety of hospitals to arrange therapeutic apheresis treatments and facilitate coordinated care. Evaluate patient medication for compatibility and safety of apheresis procedure and determine when to contact the physician. Assemble, restock, QC, and maintain supplies and equipment in a state of readiness to maintain a clean, safe, and efficient work environment. Patient Care: • Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge of the patient. • Acts as a resource for the patient and family to address concerns and questions. • Oversee the timely completion of patient care assessments and care plans. • Manages timely patient schedules to ensure facility efficiency and develop action plans for missed treatments. • Reports changes in patient status regarding any clinical, insurance, or travel and transportation issues. Consults with Charge RN and Medical Director to optimize clinical staffing. • Provides informal feedback to staff on an ongoing basis and formal feedback in the form of the annual performance evaluation during the focal merit review process. Obtains feedback and input regarding the staff performance from the Medical Director and Charge RN and acts on the feedback as appropriate. Collaborates with staff and Charge RN and Medical Director to set annual goals for staff. • Manages the department staffing through the appropriate hiring, firing and disciplinary actions. • Maintains written documentation of all disciplinary meetings in accordance with the established personnel policies and confers with Human Resources and Director of Operations regarding the nature of the disciplinary decisions. • Ensures successful execution of new hire orientation and training, and ICD-9 code training when ICD-10 applicable for new hires and works with Medical Director to ensure mandatory in-services are completed. • Ensures appropriate documentation is completed for current licensure, annual in-service and policy and procedure in-service updates. • Responsible for ensuring all facility employees receive appropriate training according to company policy including company risk management initiatives. • Provides training and guidance to facility staff members to ensure development of clinical competences, providing opportunities for professional growth and encouraging personal growth. • Collaborates with HR regarding providing information to staff pertaining to FMS/FMCNA benefits, Human Resources policies and procedures. • Participates in Corporate and Division specific employee recognition and satisfaction programs. • Maintains a close working relationship with Division and Corporate office personnel and ensures appropriate communication of FMS, Division, and Corporate initiatives, policies and procedures to facility staff. • Utilizes knowledge of FMCNA and FMS services and products to contribute to the growth of the business. • Maintains facility environmental integrity and safety. • Schedules the maintenance and repair of equipment, operating systems, and physical structure of the facility, as needed. • Monitors the security of the facility. • Coordinates inventory/supply management by reviewing monthly inventory count and approving purchase orders for necessary items to ensure cost containment, timely distribution and competitive pricing. • Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the FMS/FMS formularies. • Contributes to the development and revision of any applicable policies and procedures for the facility and the pertinent updating and maintenance of the related manuals. • Directs any necessary information gathering, as required, to support billing, billing issues and collection activities. • Coordinates and approves facility payroll. • Responsible for participating in all on-site internal and external (state and federal) surveys. • Reviews new and existing provider contracts for various vendor services, including but not limited to, insurance, laboratory, and facility cleaning. • Performs other related duties as assigned. Nursing Assistant I

Description Summary: The Nursing Assistant I performs basic nursing skills, personal care activities, and other tasks as delegated by and under the supervision of the RN/LPN as defined by the NC Board of Nursing. Responsibilities: 1. Provides quality patient care which may include but is not limited to the following: a. Measures vital signs including temperature, pulse, respirations, blood pressure, etc. b. Provides personal care such as bathing, mouth care, skin care, hair care, nail care, bed making, and dressing/undressing c. Assists with nutritional needs of patients by performing tasks such as preparing patients for meal time and feeding patients when needed d. Assist patients with elimination needs through tasks such as use of bedpan/urinal, bowel/bladder training, collection of specimens, and perineal/catheter care e. Measure height and weight f. Measure and record intake and output. 2. Accurately documents patient care as provided and according to unit specific standards. 3. Promotes effective communication between patients, teammates, physicians, and visitors. a. Promptly reports abnormal vital signs and changes in the patient’s condition to the primary nurse or charge nurse b. Communicates pertinent patient information during handoff report to other teammates c. Communicates patient or family questions or concerns in a timely manner. 4. Ensures teamwork by demonstrating cooperative, constructive, and problem solving interactions. a. Accepts delegated tasks and assignments that are within their scope of practice b. Seeks help when needed and provides help to others when available. 5. Ensures a safe, clean, and orderly environment for self, patients, and teammates. a. Utilizes proper body mechanics when turning/repositioning; transferring; assisting with ambulation; range of motion exercises; and using lifts, beds, stretchers, and wheelchairs b. Ensures patient safety through utilizing items such as side rails, call bells, and mitts/restraints c. Follows proper infection control techniques which include items such as handwashing, isolation techniques, and standard precautions. 6. Responds appropriately and promptly in emergency situations. a. Recognizes signs and symptoms of distress and promptly notifies appropriate personnel b. Obtains needed supplies/equipment in a timely manner c. Participates in CPR or other needed emergency care within their scope of practice. 7. Provides assistance and customer service to patients and visitors. a. Strives to develop rapport and trust with patients and visitors b. Adheres to behavior expectations and utilizes Carolina Care techniques c. Provides blameless apology and works toward resolution of patient/visitor concerns. WAYNE Other information: Education • Completion of state approved Level I Nurse Aide training and competency evaluation program or its equivalent. • High School Diploma or equivalent required, any higher education from an accredited college will supersede this requirement. Licensure • Current Listing on the North Carolina Nurse Aide I registry. • Certification in Basic Cardiac Life Support. Experience At least one year in a health care setting preferred.
