Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. 9. Maintains patient/employee confidentiality in the management of information. 10. Develops the plan of care based on the diagnoses and individual patient needs: 11. Records, reports, and interprets the clients' responses to the plan of care. 12. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. 13. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. 13. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT 1. Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. 2. Demonstrates accountability for own nursing practice. 3. Considers ethical issues of professional nursing practice and adheres to the code of ethics. 4. Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. 5. Serves as a resource person to nurses and other health team members. 6. Participates in peer review. 7. Incorporates research findings in clinical practice. 8. Participates in defining, maintaining, and interpreting standards of nursing practice. 11. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. 12. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. 13. Seeks opportunities to enhance professional nursing practice and influences outcomes. 14. Treats co-workers, patients and families with dignity and respect. Qualifications At least two years of nursing experience in area of specialty practice applied for. Nurse Practitioner experience of at least one year preferred Licensure: New York State License with current registration as a registered nurse. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient's condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate. Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Essential Position Responsibilities Prepare, organize, and maintain the sterile field for cardiovascular, thoracic, vascular, trauma, and related surgical procedures in accordance with established standards, policies, and aseptic technique. Anticipate the needs of the surgeon and surgical team by ensuring appropriate instrumentation, sutures, supplies, graft materials, specialty equipment, and implants are available and functioning throughout the procedure. Maintain strict aseptic technique and actively prevent contamination of the sterile field, including immediate recognition and correction of breaks in technique. Participate in sponge, sharps, and instrument counts in accordance with departmental policy, regulatory requirements, and patient safety standards. Maintain continuous awareness of patient safety, surgical flow, and procedural priorities in a high-acuity cardiovascular operating room and Level II trauma center environment. Demonstrate advanced knowledge of cardiovascular instrumentation, equipment, sterile supplies, and specialty trays, including the ability to troubleshoot and communicate needs promptly to the circulating nurse and surgical team. Receive and transfer medications, solutions, and other prescriptive items onto the sterile field from a licensed nurse to the surgeon under the direction of the surgeon and/or circulating RN. Handle, label, and prepare specimens for pathology in accordance with departmental procedures and regulatory standards. Protect patients, team members, and the environment by following infection prevention practices, universal precautions, radiation safety guidelines, and applicable perioperative safety protocols. Perform other duties as assigned to support the safe and efficient operation of the cardiovascular operating room. Qualifications Education: Graduate of an accredited surgical technology program required. Licensure and Certifications: Certified Surgical Technologist certification required. CPR/BLS required within 90 days of hire or transfer. Experience: Knowledge of sterile technique and surgical instrumentation required. Prior cardiovascular operating room experience strongly preferred. Experience supporting high-acuity surgical procedures, including cardiovascular, thoracic, vascular, and/or trauma cases in an acute care hospital or trauma center setting preferred. Work Environment: This position works primarily in a cardiovascular operating room within an acute care Level II trauma center and requires prolonged standing, walking short distances, frequent positioning, and the ability to lift, carry, push, and/or pull up to 100 lbs. The Medical Assistant/EMT Position Responsibilities Include Room patients and complete vital signs and other pertinent physical measurements. Administer medication and clinical procedures according to authorized policy and documented according to HealthPartners policy. This may include DISCUS or AIMS assessment. Review charts for the presence of pertinent information relevant to the patient’s appointment. This may include Prepared Practice work as developed by the team. Provide and document any needed and requested health educational materials to the patient. Handle patient and their records in a private and confidential manner per HealthPartners policy. Seek medical and/or nursing assistance when encountering an acutely ill patient or a patient with a changing condition. Prepare exam rooms and treatment rooms and set up for procedures. Assist provider as needed. Prepare the patient for medical interventions. Prepare charts and records for appointments as indicated by the type of clinic visit according to established practices. Check MSRs for completion and route appropriately. Prepare lab, x-ray and miscellaneous forms as requested Process drug refills according to clinic protocols Monitor station logs as assigned. Facilitate timely and accurate flow of communication within the clinic/location. Accurately receive and relay messages and test results to the appropriate person/location in a manner consistent with clinic practices and/or standards. Gather all necessary and appropriate information from a caller and relay information to the appropriate provider for necessary follow-up. Inform reception desk of provider’s schedule status and assist in coordination of urgent and non-urgent provider schedule changes to ensure patient and provider needs are met. Perform patient correspondence when requested and assist patients in completing forms when appropriate. Prepare and sterilize instruments and equipment according to procedure and OSHA standards. Complete scheduled cleaning requirements for designated work areas. Document completion of work to comply with infection control and OSHA standards Implement other infection control procedures as needed and adhere to protective clothing requirements. Ensure clinic supplies are adequately stocked and well organized. Process and deliver department inventory requests in a timely manner. Routinely check expiration dates and remove or replace outdated supplies. Maintain prescription sample supply cabinet as per clinic protocol Ensure all equipment is properly maintained and adequate records are kept. Ensure inventory of instruments and equipment is current. Ensure instruments and equipment are calibrated as needed and maintained in proper working order. The Medical Assistant, EMT and LPN position requires the ability to sit or stand for prolonged periods of time. Must possess speech, hearing, and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently.
