Registered Nurse RN Preceptor – Home Health Services Home is where the heart is! JOB PURPOSE: The RN Preceptor is responsible for activities related to orientation, onboarding, and education of new RN partners. The RN Preceptor is intended to provide the support and guidance needed to integrate new RN partners to PruittHealth @ Home and ensure new RN partners become independent and clinically competent members of the PruittHealth @ Home team. When not precepting, the RN will spend 100% of their time taking on full patient load. KEY RESPONSIBILITIES: Precepting Duties • Serve as the primary educator for all new RN partners within a specified region • Typically, will begin precepting in week 3 of onboarding and orientation, following clinical orientation. • Teaches, demonstrates, and supervises clinical activities and documentation to aid in the development of preceptees skills to ensure proper patient care. • Primarily dedicated to precepting. When not precepting, will spend 100% of time taking on full patient load. Precepting rotations will average eight weeks (depending on new hire’s experience) and the number of rotations each year will fluctuate based on the number of incoming new hires within the Preceptor’s home health agency (HHA). • Work with various agency stakeholders to assess and progress the preceptees’ experience and comfort level. • Participates in the evaluation of clinical skills and medical knowledge base through the following mechanisms: O Teaching in the patient setting O Direct supervision O Observation O Provides timely and ongoing feedback to the Preceptee each week in a candid summary discussion. O Meets with the Preceptorship Coordinator weekly to provide feedback on the program, which will directly inform and drive improvements to the precepting process / experience. Clinical Duties • Provides advanced nursing care & interventions to home health patients according to state & industry practice standards (on-call basis, as needed) • Communicates with RN supervisors, Clinical Managers, and patient care teams (as appropriate) of any changes noted during care. • Demonstrates advanced knowledge of home care and competency in discipline for specific patient care skills, required for the provision of patient care. Technical Duties • Assess, monitor, documents and reports progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines. • Completes documentation timely, accurately, and at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination, etc.).
