This position also performs clerical duties, acts as receptionist, maintains medical records and communicates with all departments. Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Triage patient phone calls and troubleshoot medication management issues Documents nursing observations, interventions, and patient’s response to treatment in patient records. Demonstrated competency in performing venipuncture/administering shots for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN). Accurately performs and records vital signs, weight and height, point of service blood glucose testing, and promptly reports abnormalities to the medical provider or Registered Nurse (RN) on duty. Utilizes appropriate techniques when collecting routine urine specimens and specimens for C/S. Appropriately instructs patients on clean catch specimens. Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure. Verbalizes knowledge of and adherence to OSHA standards for blood borne pathogens when handling blood and body fluids. Maintains standard precautions in the provision of patient care. Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis. Prepares and processes requisitions for routine supplies in accordance with agency procedures. Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager. Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports. Copies, prepares records, and stores and maintains appropriate records. Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions. Conforms to policies and regulations governing patient rights and confidentiality of information. Adheres to protocol for patient flow (1. Reception, 2. Registration, 3. Labs, 4. Escort to Provider). Perform all other related duties as assigned by management. Qualifications Required Education & Qualifications: Graduate of an accredited school of Practical Nursing. Currently registered in New York State as a Licensed Practical Nurse (LPN). Must have excellent communications skills Demonstrated competency in performing venipuncture for collection of blood specimens. Overview Pay Rate: $40/hour This is a direct service position providing individual medical support and education to individuals in assigned programs.To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals.Hours assigned by supervisor; on call responsibilities. Responsibilities •Assess and coordinate healthcare services for participants. Institute appropriate follow up and communicate all medical issues to: Residential Manager, Health Care Director or program residential RN, Area Director, and any other appropriate SHCS staff.•Assist and transport individuals in designated programs with their medical appointments as recommended by their physicians and DDS standards.•Document all nursing interventions and changes affecting the consumer in appropriate areas –TIER, progress notes, and staff communication log.•Review ISPs for current knowledge of programming in self-medication, behavior plans, hygiene skills, and physical therapy plans.•Maintain current medication and treatment orders noting any side effects/adverse effects and report to appropriate people: M.D., Residential Manager, Health Care Director or program residential RN, and other designated SHCS staff. Assist in training staff in medication policies and procedures.•Maintain health records according to DDS standards assuring that necessary documentation is up to date.Ensure that information in communicated to staff-residential and day program regarding recommendations and treatments.•Monitor for the observance by all maintaining safety.(A) Meet with Health Care Director or program residential RN for supervision and direction as needed to ensure appropriate medical health care is provided for participants.(B) Attend staff meetings to present all current medical/health issues and concerns to team as well as upcoming medical appointments.(C) Attend other designated mandatory in-services.To implement procedures and services including medical interventions, or additional duties, as assigned by the Health Care Director/program residential RN. Job Description Situated at the base of the Rocky Mountains, Balfour Senior Living in Louisville, CO at Balfour Retirement Community is seeking talented and team-oriented associates. The CNA meet community residents’ needs by helping with activities of daily living. Responsible for the delivery of quality care by encouraging independence and ensuring each resident’s physical, social, cultural, and intellectual well-being. Schedule: Full-Time | Tuesday-Saturday 10:30pm-6:30am PAY: $23-25/hr plus evening differential rate + $1.50 evening differential rate after 6:30pm! What will I do every day? Demonstrate Balfour Service and Operational Values in every encounter• Performs nursing assistant duties such as intake and output, measuring height and weight, vital signs, specimen collection, catheter care, range of motion exercises, etc• Documents residents’ information accurately and on a daily basis on the appropriate forms (i.e. intake andoutput forms [if ordered], vital sign sheets, and B.M. record, etc.)• Provides all personal care for residents, including bathing, showering, shampooing, oral hygiene, skin care, hair care, nail care, dressing, shaving, toileting, transferring, turning and positioning, ambulating, and transporting• Recognizes changes in the residents’ behavior and conditions, and cares for their environment• Assists residents with nutritional needs, including serving, feeding, providing proper fluid requirements, andoffering substitutions when appropriate• Monitors food and fluid intake and offers/provides snacks and fluids between meals• Cares for the dying resident and provides postmortem care• Adheres to strict infection control guidelines including hand washing and universal precautions• Reports any and all exposures to infection in accordance with OSHA and Balfour guidelines, policies, and procedures• Performs all duties in a safe and efficient manner. Duties may include placing residents’ call light within easy reach, applying physician ordered restraints (i.e. tabs alarm, lap buddy, etc.).• Observes and reports residents’ condition changes to the care manager immediately and exchanges resident information with the care manager on an ongoing basis• Is consistently responsive to residents’ needs; responds to any and all call lights and acknowledges residents’ and families’ concerns and initiates action• Provides privacy and maintains confidentiality and ensures the residents’ right to make personal choices• Attends in-service education programs mandated by Balfour, state, and federal guidelines What will I need to be successful in this role? • State certification and graduation from a state approved nursing assistant training program• Ability to work effectively as part of a team• Ability to communicate openly, honestly and responsibly with others • Ability to maintain a customer focus treating all customers with respect and integrity• Ability to read thermometers, charts, and testing equipment• Ability to read, speak, and understand the English language• Demonstrated time management, ability to prioritize under competitive demands Special Requirements/Certifications I may need?
