The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. How does scheduling work? Scheduling is observed in a four-week period. The nurse manager inputs the schedule and pays close attention to staff requests including desired schedule patterns and vacation times. Job Objectives: Assumes direct responsibility and accountability for, and authority of, the nursing care of all patients assigned. Responsibilities Assesses and identifies the patient's physiological and psycho-social problems and needs. Develops an individualized plan of nursing care for each patient in which goals, in terms of measurable outcomes, are stated. Ensures that nursing care plan is consistent with the medical plan and adheres to accepted current standards of care and practice. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Obtains and documents a complete nursing admission assessment, including physiological and psychological factors to develop an interdisciplinary plan of care within eight hours of admission. Performs and documents an accurate systems assessment at the beginning of each shift. Responds to diagnostic data and initiates appropriate action or intervention. Assesses patient education needs on admission and daily. Assesses psychosocial needs specific to patient's age, growth, and development. Initiates an interdisciplinary plan of care within eight hours of admission. Establishes priorities for patient care based on patient needs as evidenced by completion of assigned duties within shift. Verbalizes understanding of patient diagnosis and interdisciplinary plan of care. Safely and correctly initiates and documents nursing interventions in emergency situations. Performs 24-hour chart check at the beginning of each shift. Administers, observes, and documents intravenous therapy according to hospital policy. Administers medications within 30 minutes of scheduled time. Observes and documents effectiveness. Recognizes and reports adverse reactions immediately. Provides individualized instructions each shift to patient/significant other regarding the plan of care. Demonstrates safe and correct use of unit specific equipment. Ensures that each patient's general hygiene needs are met daily. Performs pre-operative and post operative care and instruction for each assigned surgical patient during shift. Adheres to standard precautions including hand hygiene guidelines at all times. Documents specific patient valuables on designated form when received and immediately arranges for appropriate storage. Performs dressing changes using aseptic technique. Collects and labels specimens per policy when ordered by physician. Performs bedside glucose monitoring according to policy. Administers internal feedings via ng tube, g tube, and/or j tube. Reports all abnormal findings immediately to physician. Initiates wound and skin protocol upon identification of risk. Performs ostomy care to include irrigation, appliance usage, and reinforcement of teaching. Inserts intermittent or indwelling urinary catheter using aseptic technique. Administers oxygen therapy according to physician order. Transfers patients in/out of the unit in an efficient manner, by obtaining a complete and correct set of physician's orders. Completes documentation of transfer summary. Ensures appropriate completed referral forms accompany intra-facility transfers, including giving/receiving of report. Demonstrates safe and correct use of restraints as per policy. Documents/reports patient responses or outcomes resulting from: nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education. Evaluates and documents patient's progress towards established goals/objectives every shift. Updates/revises patient care problem list and individualized patient care standards based on patient/family responses, progress towards goals, and/or change in medical status every shift. Evaluates patient length of stay and related transfer/discharge planning needs based on anticipated problems/needs, internal or community resources, and support systems. Obtains 100% mandatory hospital and unit specific in-services annually, prior to evaluation date. Plans, presents, and documents unit in-service as required by level. Documents competency in clinical skills annually. Attends staff meetings as required. Uses established lines of communication for resolution of problems and interpersonal conflict. Qualifications: Current Licensure in the state of Florida and Basic Life Support and Current Advanced Cardiac Life Support required. CPR Certification and Chemotherapy Immunotherapy Course and Certificate required (must be obtained within 3 months of hire or transfer). Specialty or National/Professional Certification preferred.
