Educate patients and families on disease management and care plans. Document patient visits and maintain accurate medical records. Collaborate with interdisciplinary team to ensure quality patient care. Qualifications: Graduate of an accredited practical nurse or vocational nursing program (LVN/LPN). Minimum 1 year of experience, preferably in home care or med/surg. Current state license and CPR/BLS certification. Strong communication and problem-solving skills.
