All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. ESSENTIAL DUTIES & RESPONSIBILTIES: By following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following: • Maintain a healthy, safe environment. Providing nutritional and environmental support. • Assist with personal hygiene, toileting, dressing, walking and feeding the client. • Position the client when instructed to do so. • Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor. • Remind the client to take medications. • Assist with payment of bills ONLY when authorized to do so and it is documented on the assignment sheet. • Run errands when authorized to do so. Documentation on the Aide Charting Sheet daily. • Report any important information or changes in the client's condition to the agency and/or the nursing supervisor when necessary. • Use of medical supplies and equipment when appropriate. • Emotional support and the development of a trusting relationship between the aide and the client. • Encouragement of client independence.
