Assist clients with following a written, special diet plan and reinforcement of diet maintenance, which is provided under the direction of a Physician and as identified on the care plan. Escort clients to medical facilities, errands, shopping and outings as specified in the care plan. Assist in basic client transfers providing the client has been assessed as being capable of ambulating without assistance; and/or, providing another trained caregiver (including family) is involved in the transfer. Assist clients with communication by writing or typing correspondence for them or researching information for them. Provide companionship, friendship and emotional support. Talk, listen, share experiences, play games/cards, read to client etc. Help keep clients in contact with family, friends and the outside world. Provide transportation to medical appointments, grocery store and errands. Accompany clients to recreational and/or social events. Assist with plans for visits and outings. Participate on the Care Team by providing input and making suggestions. Ensure service is delivered in accordance with all relevant policies, procedures and practices. Monitor supplies and resources. Evaluate the program and make recommendations to it, as indicated. Follow the written care plan. Carry out duties as assigned by the Supervisor. Observe clients and their environments and reports unsafe conditions to Supervisor. Observe clients and their environments and reports behavior, physical and/or cognitive changes and/or changes in living arrangements to Supervisor. Complete and maintain records of daily activities, observations, and direct hours of service. Attend orientation, in-service training sessions and staff meetings. Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. What You’ll Do As a Pediatric Home Care Specialist & Acute Care Registered Nurse, you serve as the clinical bridge between home‑based care and virtual acute care. You will deliver high‑quality, family‑centered care in the home and community while flexing into virtual acute care nursing to support urgent pediatric needs. In this role you will: Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care. Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care. Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists. Conduct and document in‑home assessments; perform procedures such as ear irrigation, point‑of‑care testing, IV placement and lab draws, wound care, and medication administration. The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. The completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of a plan of care that takes into consideration client’s home environment and social support system. - Complete in-home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. The reviews consist of screening all documents related to the request for service and may include contacting the client’s physician or other resources for additional medical documentation and information as necessary. - Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation. - Prepare documentation for and testify at Fair Hearings and at other legal proceedings to explain clinical findings and determinations regarding the plan of care. - Provide consultation to HRA Medical Review Team and social service staff at HRA’s request. - Complete self-audits of State approved assessments and medical review of charts. - Work toward obtaining PRI/screen certification and conduct assessments of clients transitioning to a Nursing Home Facility.
