Licensed Practical Nurse/Licensed Vocational Nurse This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. Administer medications and provide documentation as well as oversee and monitor staff members who administer medications. Provide prescribed medical treatment by physician’s orders and personal care services. Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition. Provide training to staff members including universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care. Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 3pm-11:30pm, Tues-Sat. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Now Hiring - Certified Home Health Aide (CHHA) - North Team This Certified Home Health Aide will be responsible for, but not limited to: Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
