Provides appropriate support at time of death and perform bereavement assessment. What is Required? Maintain regular communication with Hospice director, Medical Director and attending physician concerning caseload. 4. Obtain data on physical, psychological, social and spiritual factors that may influence patient/family status and incorporate that data into the plan of care. 5. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. 6. Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support. 7. Accept responsibility for coordinating physical care of the patient by teaching of the patient by teaching primary caregivers, volunteers, employed caregivers (LPN’s, hospice aides), or by providing direct care as appropriate. 8. Meet regularly with hospice nursing staff, and attend interdisciplinary team meetings to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth, and facilitate issues from caseload for full staff discussion, consultation and evaluation. 9. Supervise and document hospice aide plan of care every 2 weeks and supervise and document the practice of the LPN in the provision of patient care as delegated. Other Duties: 1.
