Job Overview The IPA RN Inpatient Case Manager will support daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facility (SNF) in-person as well as telephonically. Bachelor’s degree in social work, nursing, or another health or human services field with the appropriate licensure preferred. Minimum of 1 year of experience in public health nursing, acute care, case management, and/or home health care is preferred. Minimum of 1 year of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care is preferred.
