Teamwork: To facilitate team care, the Triage Nurse sits in the common area with the physicians and other clinical support staff assigned to her/his team. • Ensures requests for medication refills, test results, and other patient concerns are completed. • Promotes team-based care by utilizing the Eskenazi Health Model of Excellence (EME) tools. • Assists clinical teams manage patient panels to meet quality metrics by out reaching to patients who need appropriate disease specific or age-specific screenings. • Collaborating with the provider and other clinical support team members to ensure that patients receive appropriate and adequate instruction regarding new diagnoses, new medications, as well as wellness recommendations. • Conducts nurse visits for patients assigned to the team Initiative & Growth: Develops goals for professional growth of self and team members. •Demonstrates a self-directed work effort aligned with team goals. •Complete Eskenazi Institute for Leadership training within 18 months of assignment. • Performs quality improvement activities: identify processes needing improvement; develops and implement improvement plan, performs pre- and post-intervention audits within 18 months of assignment. Technical Skill: Demonstrates technical ability, understanding of job expectations and compliance with organizational policies and procedures. •Consistently tracks, organizes, and prioritizes the workload with the ability to sort through routine and stat data while keeping both the provider and the patient informed. •Charts assessments, interventions, and patient response concisely, descriptively, and legibly. •Utilizes established protocols to manage phone and walk-in triage of patients to identify the level of care needed, assign a triage priority, and implement the appropriate plan of action. •Use of critical thinking skills in the care of the patients/families and to resolve issues within the team. Job Requirements • Registered Nurse (RN) with state licensure. • Current CPR/BLS certification. • A minimum of three years progressive professional experience as a nurse with at least 1 year in an ambulatory setting preferred. • Fully proficient verbal and written communication skills. • Experience with adult and/or pediatric IV and procedure skills preferred. Initiate the Plan of Care and makes necessary revisions as patient status and needs change. 4. Provide education to patients and family members on proper home health procedures and strategies. 5. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. 6. Administer medications and treatments as prescribed by the physician. 7. Provides health care instructions to the patient as appropriate per assessment and plan of care. 8. Identifies discharge planning needs as part of the care plan. Prepares patient and family for discharge. 9. RN acts as Case Manager when assigned by Director of Nursing and assumes responsibility to coordinate patient care for assigned caseload. 10. Coordinate with physicians, physical therapists, occupational therapists, MSWs and other individuals in the patient care plan. Perform a supervisory visit with CNA (when part of the care plan) once every two (2) weeks. Communication: 1. Prepares clinical notes and updates the primary physician when necessary. 2. Communicate with the physician and office regarding the patient's needs and report any changes in the patient's condition; obtain/receives physician's orders as required. 3. Communicate with community health-related persons to coordinate the care plan. 4. Registered Nurse (RN) reports with home office on any updates, concerns and issues regarding the paient.
