As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. This role reports to the Center Medical Director and works closely with operational leadership. A minimum of five to seven years demonstrated progressive nursing leadership or education roles required. Must have and maintain credentials required for clinical area, which may include ACLS, BLS, and NRP. (If, NRP is required for clinical area, must obtain instructor status within the first year of employment). Certification in specialty area is required or must be achieved within the first year of employment. Membership in professional nursing organization required or must be achieved within the first year of employment. Excellent human relations, verbal, and written communication skills required. Knowledge of word processing, spreadsheets, and presentation computer software required. Teaching experience is highly desirable. Job Summary The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for orthopedic surgical patients. Is responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services, including coordinating patient care throughout the continuum in collaboration with the multidisciplinary team. The Nurse Navigator will serve as a clinical resource with expertise in orthopedic care management and will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population and will provide expert nursing care, which includes: direct clinical practice, consultation and education. Duties & Responsibilities Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship. Systematically and continually performs the functions of assessing, planning, implementing, and evaluating the care according to the nursing process and Nursing Society Standards of Practice. Provides education and information to the patients and their family, helping to make the care seamless, continuous and comprehensive.
