Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Schedule: 36-hours per week, FT Shift: Days Location: Javon Bea Hospital—Riverside, Rockford, IL Pay Range: $17.57 - $25.48 ESSENTIAL DUTIES AND RESPONSIBILITIES Provides patient care as assigned and per routine as defined by educational scope, according to standards of care, shift routines, and policies and procedures. Answers call lights in a timely and appropriate manner. Documents care provided according to standards. Maintains a clean work and patient environment. Maintains supplies, including scanning, stocking and ordering new supplies and disposing of expired or damaged supplies. Floats to various units based on identified patient care needs. Participates in department or unit performance improvement activities. Training/precepting new partners in collaboration with unit educator Blood Glucose Monitoring (BGM) with lab personnel Bladder scanning procedure reviewed Bladder scanner training complete (check off sheet for competency) Attaching patient to remote telemetry or bedside cardiac monitor Managing and reporting collection devices: Colostomy, ileostomy or other fecal management systems: care and output measures Indwelling catheters: care and output measures Nasogastric tubes to suction Surgical drains under direction of supervising RN Competent in completing vital signs: Manual process for blood pressure and required reporting of abnormal values to supervising RN Discontinue invasive devices under the direction of the supervising RN: Indwelling (Foley) catheters Nasogastric tubes Peripherally inserted intravenous catheters Similar devices as appropriate EDUCATION AND/OR EXPERIENCE Completion of a CNA program. CERTIFICATION/LICENSURE Current CNA certificate in applicable state. Approved CPR or BLS certification required within 90 days.
