Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients and more. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information. Performs other related duties as assigned. Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. Master’s degree in nursing or related field. Distributes food/water and feeds residents. Assists with repositioning and transferring residents. Ambulates and transports residents. Records and totals fluid intake and output. Weighs residents. Documents on flow sheets. Assists with admission, discharge and postmortem care. Responds to call lights in a timely manner. Performs perineal care and uncomplicated colostomy care. Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse. Takes and records vital signs. Makes beds and keeps resident’s units clean and in order. Prepares room for housekeeping after resident discharge. Skill / Experience Requirements . Current NYS CNA Certification Required. Previous Long Term Care experience a plus.
