Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Description The Clinical Nurse for the Mount Sinai nursing float pool is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients on a variety of assigned units. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Description The Per Diem Assistant Nurse Manager for Labor & Deliver is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager on a per diem basis. Description The Assistant Nurse Manager for the Emergency Department is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Systematically and accurately assesses nursing needs of the patient in order to detect pertinent changes in the clinical picture. Selects, plans, evaluates, and documents care based on this assessment. Assesses, intervenes, and documents appropriately to meet patient's educational, psychosocial, and discharge planning needs. Initiates appropriate referrals to other departments, services, and/or outside agencies for patient care. Gives comprehensive patient care. Initiates and follows through with patient and family teaching. Consults with unit Nursing Leadership regarding significant changes in patient's condition. Communicates relevant patient care information on shift-to-shift report. Utilizes patient care standards in clinical practice. Collaborate with all members of the health care team. Documents pertinent information regarding patient's response to treatment and progression toward goals in patient's chart. Completes Admission/Transfer Summary Sheet and/or Discharge Summary. Transcribes physician's orders in patient charts accurately and timely (when Ward Clerk is not available). Seeks clarification of physician's orders that are unclear or inappropriate. Interprets clinical data and notifies the physician promptly if immediate changes in management are indicated. Administers intravenous medications in compliance with policies and procedures. Titrates medicated infusions as per physician's prescriptive, protocols, and standard. Transports patients to operating room and from post anesthesia care unit. Checks to ensure that emergency equipment is on hand and in good working order. Recognizes and reports malfunction or failure of equipment used in the area to Biomedical Engineering; takes appropriate action to repair/replace such equipment (e.g., ventilators, cardiac monitors, defibrillators, suction apparatus, pumps, doppler, O2 tank, and pacemaker). Checks policy and procedures on proper handling of sharps. INDIRECT PATIENT CARE ACTIVITIES Participates in interdisciplinary patient care rounds by presenting findings on patients. Apply principles of infection control and safety. Assists in the orientation of new personnel. Provides clinical supervision of LPNs, Certified Nursing Assistant, Ward Clerks and other ancillary staff.
