The primary responsibilities of the Medication Technician is to: 1) administer medications to the residents in a safe, accurate and timely manner and 2) provide physical and psychosocial care to all residents in a manner that promotes dignity, choice and individuality. Position Highlights: 1. Satisfactory completion of medication technician skills competency checklist. 2. Must obtain required medication technician training certificate. 3. Experience working in Assisted Living preferred. 4. Must be able to read, write and speak the English language. Understands and practices universal precautions with all patient. Performs all tasks/procedures included on assignment or reports to charge nurse any tasks not completed. Assists in the preparation for admission of patients. Assists in and accompany patients in admission, transfer and discharge procedures. Provides post-mortem care. Provides morning care, which may include bed bath, oral hygiene, combing hair, back care, dressing patients, changing bed linen, cleaning overbed table and bedside stand, straightening room, and other general care as necessary throughout the day. Position Summary Assist in the care of medical-surgical patients with a primary focus on activities of daily living under the direction of a nurse. The CNA will perform and document vital signs, intake and output, glucose monitoring and other patient care duties Education High School Diploma - General Studies - Preferred Experience 1 Year - Required License & Certification Professional Certification - Required Core Job Functions Proper safety procedures are utilized to ensure patient and staff safety are utilized. Verbally reports abnormal vital signs or other abnormal assessments to a nurse. Follows policies and procedures and physician's orders. Performs bedside glucose testing. Accurately records results according to standard operating procedure. Quality Control (QC) and maintenance performed and documented. Utilize interpersonal and therapeutic communication skills when interacting with patients, families, and members of the Provider and RN. Communicates relevant, accurate, and complete information. Under the direction of the RN transports patients to other patient care areas Training is completed on all safe handling devices. Assesses the patient to determine the most appropriate lift equipment to use on a specific patient. Lift assist devices are used on all needed transports. Call lights and phones are answered in an efficient and professional manner and messages are communicated to the appropriate person. Work area is kept organized and clean. Adheres to safety policy wearing protective equipment while cleaning and reports any needed repairs directly to charge nurse. Follows Isolation Precautions per policy. Trash and linens are bagged per policy and placed in soiled utility room, cleans and sanitizes all equipment and as needed to include stretchers, wheelchairs, carts, IV poles etc.

Conducts comprehensive holistic wellness and illness assessments and employs evidence-based clinical practice guidelines in care of patients. Provides leadership for collaborative, evidence-based revision of diagnoses and plans of care, to improve patient outcomes. Develops evidence-based clinical interventions and systems to achieve defined patient and system outcomes. Complies with regulatory guidelines for work area and professional practice standards. Assists with data collection, trouble shooting, and educating staff of performance improvement indicators. Utilizes available safety equipment and processes for patient care. Care plan developed and implemented with other health care team members. Is a resource for others in the delivery of timely professional communication with physicians regarding Situation, Background, Assessment, and Recommendation format. Is a resource for assuring consistent rounding and communicating including present on admission indicators. Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care.

Position Summary Utilizing the Nursing Process of assessing, planning, implementing and evaluating, the Staff Registered Nurse is responsible for delivery of patient care; managing assigned personnel, and promoting collaboration among the multi – disciplinary health care team. Education Associate Degree in Nursing - Required Experience 1-2 Years Acute direct patient care - Preferred Two years Medical/Surgical and/or Critical Care in a hospital within the last five years preferred. License & Certification Registered Nurse License with State of Practice - Required If not currently nationally certified in the area of practice, expected within 2 years of hire. Basic Life Support (BLS) - Required Advanced Cardiovascular Life Support (ACLS) - Required in 90 days from hire Core Job Functions Assessment and documentation of the physical, psycho-social, growth, Functional Independence Measures (FIM) scores, and developmental dimensions of the patient. Includes patient and other health care team members in developing and implementing plan of care using the medical/ nursing diagnosis. Customizes and prioritizes plan of care and helps to develop problem list as needed. Evaluates long and short term goals including communication of plan of care and evaluation to patient, family, provider as appropriate. Teach back performed with patients to facilitate patient outcome improvements by increasing knowledge and skills needed to improve healthy behaviors. Elicit information using effective counseling techniques. Identify factors that might affect communication or learning. Conducts comprehensive holistic wellness and illness assessments and employs evidence-based clinical practice guidelines in care of patients. Provides leadership for collaborative, evidence-based revision of diagnoses and plans of care, to improve patient outcomes. Develops evidence-based clinical interventions and systems to achieve defined patient and system outcomes. Complies with regulatory guidelines for work area and professional practice standards. Assists with data collection, trouble shooting, and educating staff of performance improvement indicators. Utilizes available safety equipment and processes for patient care. Care plan developed and implemented with other health care team members. Is a resource for others in the delivery of timely professional communication with physicians regarding Situation, Background, Assessment, and Recommendation format. Is a resource for assuring consistent rounding and communicating including present on admission indicators. Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care.
