About this Job: General Summary of Position Facilitates the inpatient admission of appropriate rehabilitation patients to MedStar National Rehabilitation Network (MNRN) by obtaining accurate clinical data through performing on-site and record/ telephonic patient evaluations. Individuals in this role are responsible for educating the referring staff of community hospitalspatients family members and external payers regarding the effective and efficient utilization of MNRN program services and resources. Primary Duties and Responsibilities Performs on-site record and telephonic pre-admission screening activities by gathering clinical information to support the evaluation of patients for admission to the inpatient rehabilitation program. Completes assessment/ evaluation within 24 hours or in one working day of the referral (documents exceptions within the electronic medical record). Reviews clinical information in patient's medical record and performs physical assessment of the patient within in the scope of LPN practice. Assists in compiling summaries and referral documentation for review by licensed provider responsible for determining medical stability and admission appropriateness. Reviews clinical data/assessment with MNRN manager and/ or physician for determination of medical stability and appropriateness for admission and notifies referral source of MNRN decision.
