He / she ensures the highest quality and safe delivery of patient care while maintaining compliance with professional nursing, AAAHC, and all Federal and State standards of care. The RN cares for all ages from 16 to 80 years old. Required Skills: Graduate of an accredited RN School of Nursing. At least 1 year experience as a RN in hospital Medical/Surgery or ICU unit. Currently licenses in the state. BLS Current ACLS preferred or included in education plan Ability to make decisions based on nursing judgment. Able to prioritize and organize workload to meet needs of patients and unit. Strong communication skills, verbal and written.

Your Role & Responsibilities Provide comprehensive care to seniors in accordance with clinical standards. Collaborate daily with your care team to ensure coordinated, high-quality care. Manage referrals, hospital/SNF coordination, DME, and home health services. Participate in daily huddles. CAs tend to lead the huddles Document care accurately and efficiently with support from quality-based coders. Participate in on-call rotation and support growth initiatives at your center. Ensure compliance with all licensing and accreditation requirements. What We’re Looking For Required: Master’s Degree in Nursing or completion of a PA program with board certification. Active, unrestricted NP or PA license in the state of practice. Minimum of 1 year NP or PA experience Strong communication and interpersonal skills. Passion for value-based care and integrated team-based models. Basic computer proficiency (EMR, email, etc.). Use your skills to make an impact Preferred: Experience in value-based care, primary care or Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers Familiarity with HEDIS quality indicators and Medicare guidelines.
