Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person health. Under the supervision and direction of the Clinical Practice Manager, the Registered Nurse is responsible for providing services which consists of an evaluation and assessment of medical and psychiatric status as an integral part of the interdisciplinary team. The Registered Nurse also provides direct patient care in the departments of primary care and psychiatry. Essential Duties & Responsibilities: Provides direct patient care, carrying out orders given by my medical provider, which may include obtaining medical histories, obtaining vital signs, performing office lab testing and phlebotomy/ venipuncture, EKGs, wound care, and which includes but is not limited to medication administration. Conducting nursing visits or educational groups as ordered by the medical provider Maintains accurate, detailed reports and records. Records and reconciles patients' medical information. Monitors, records, and reports symptoms or changes in patients' conditions, assess efficacy of prescribed medications. Ensure gaps in care for screenings, chronic care tests (such as HgbA1c for diabetes), BMI, vaccinations, are completed in timelines established by best practice. Facilitates effective crisis intervention. Perform safety assessments and triage for patients in crisis. Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans. Modifies patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Maintains an active 10- patient panel for Complex Care Management (CCM). Performs patient triage of acute conditions in-person or over the phone. Refers patients to specialized health resources or community agencies furnishing assistance. Performs other duties as required. *All agency staff are required to attend all mandatory department/agency meetings and trainings Requirements Education &/Or Experience: Graduation from an accredited nursing program. Current State of Connecticut RN licensure required. One (1) year medical experience in a public health setting required. Current Connecticut RN License required. 2 years of community health, addiction medicine and/or psychiatric nursing required. Basic Life Support (BLS)/ CPR certification. EPIC experience preferred, but not required. Bilingual (Spanish Speaking) preferred, but not required.

Perform opening and closing duties, which includes cashing out, end-of-day communication, and securing the building. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed. RN Registered Nurse (PER DIEM) Masonicare at Greenridge Place - Rocky Hill, CT Per Diem Under the direction of the Supervisor of Assisted Living, assumes the professional responsibility and accountability for a group of residents, providing and coordinating care which is focused on achieving the residents optimal functional level of health within the framework of supportive and restorative care, health counseling and teaching, case finding and referral. Understands the Mission, Vision and Values of Masonicare, and Masonicare Assisted Living. Essential Duties and Responsibilities: 1. Performs initial and ongoing nursing assessments utilizing the nursing process to establish a comprehensive goal directed resident plan of services. 2. Develops, implements and evaluates resident plan of services. 3. Transcribes, implements and evaluates physician orders in a timely manner. 4. Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs. 5. Provides coordination of the plan of services and maintains continuity of care with other professionals. 6. Documents accurate, pertinent and comprehensive information in a timely and concise manner. 7. Conducts supervisory visits of resident care assistants, companions and homemakers as required. 8. Arranges and participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. 9. Perform other duties as assigned. 10. Assessments, completed as often as necessary based on client's condition but not less frequently than every one hundred and twenty (120) days, and prompt action when a change in the client's condition would require a change in the client's service program. 11. Referral to appropriate professionals or agencies, whenever the client's condition necessitates, including the provision of current clinical information ensuring that if the client's condition is no longer chronic and stable, services of a licensed home health agency are engaged or other appropriate arrangement are made. 12. Planning for clients who shall no longer receive or require the services of the assisted living services agency. 13. Implementation or delegation of responsibility for the availability of nursing services on a twenty-four (24) hour basis. *This may include On-Call responsibilities. 14. Nursing services, which shall include, but not necessarily be limited to: a. client teaching b. wellness teaching c. health promotion d. disease prevention e. Responsible for medication administration and delegation of supervision of self-administered medications as specified in subdivision (4) of the subsection. 15. All other duties as assigned.
