Neonatal Nurse Practitioner

Summary: Provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty board certification. Operates in accordance with Section 4723.431 of the Ohio Revised Code and rule adopted by the Board of Nursing for Advanced Practice Nurses, the national board certifying agency, and maintains a Standard Care Arrangement with a collaborating physician(s) on the medical staff. Responsibilities: 1. Performs health assessments and physical exams Identifies significant health and medical risk factors 2. Develops a diagnosis by analyzing and synthesizing collected data. 3. Orders, interprets and evaluates tests and studies to assess clinical problems and health care needs. 4. Performs diagnostic or therapeutic interventions consistent with board certification, state law and organizational medical staff privileges. 5. May prescribe and monitor medications as allowed by prescriptive authority through Ohio Board of Nursing and hospital policy. 6. Develops patient education plan. 7. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care. 8. Performs admission history & physical, document progress notes, complete discharge summaries as required. 9. Independently and in collaboration with the physician, develops treatment plans along with the patient and significant others. 10. Provides consultations and requests consults as directed and in accordance with hospital bylaws. 11. Assists with and facilitates discharge planning. 12. May serve as resource for staff/residents and clinical preceptor for APN students. 13. Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. 14. Practice with a variety of specialty-based populations including neonatal, pediatric, and adult 15. Practices in any setting in which patients require complex monitoring and therapies, high intensity nursing intervention, or continuous nursing vigilance within the range of high-acuity care. 16. Participates in quality monitoring through the review of records and treatment plans for patient outcomes on a bi-annual basis to assure quality care and per ORC requirements. 17. This review is completed via the Focused Professional Performance Evaluation (FPPE) and Ongoing Professional Performance Evaluation (OPPE) for Advanced Practice Providers as part of Joint Commission requirements. 18. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. 19. Participates in quality monitoring through the review of records and treatment plans for patient outcomes on a bi-annual basis to assure quality care and per ORC requirements. 20. This review is completed via the Focused Professional Performance Evaluation (FPPE) and Ongoing Professional Performance Evaluation (OPPE) for Advanced Practice Providers as part of Joint Commission requirement. 21. Other duties as required. Other information: Technical Expertise 1. Excellent critical thinking, self-motivation and interpersonal communication skills essential. 2. Experience working with all levels within an organization is preferred. 3. Must be proficient in word processing, spreadsheet and database use, and other computer software programs for data analysis, and presentations. Education and Experience 1. Education: Master's or Doctoral Degree preferred. 2. Certification: National Certification as a Nurse Practitioner by a nationally accredited organization. Must have current certificate of authority and certificate to prescribe (or eligible) by Ohio Board of Nursing. ACLS or PALS may be required based upon practice setting. Must obtain credentialing as an active member of medical staff and appropriate medical staff privileges. Current Health Care Provider BLS training is required. Individuals hired without an MSN or DNP and those hired without a certificate to prescribe must actively pursue and obtain a MSN or DNP degree/certificate to prescribe. 3. Licensed Practical Nurse (LPN)-Hospice PT/PRN

Care Comfort Hospice is looking for Part Time/PRN LPN Hospice Nurses. ALL SHIFTS AVAILABLE! FLEXIBLE SCHEDULE! I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · provide Nurse Case Manager with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making referrals for home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. · reports to DON as patient condition changes, changes in the patient condition that require change in level of care. III. Orientation and Training Care Comfort Hospice will provide orientation and training, as necessary, to assist the Hospice Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Hospice Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing.
