Responsible for providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. The Mobile Hospital at Home Registered Nurse provides hospital-level acute care in a patient’s home within a defined radius around designated hospitals as part of the Advocate Hospital at Home team, which may include the following: Physicians, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Physical Therapists, and other operational staff according to the Hospital at Home Care Model. We’re Looking For: Completion of an accredited or approved program in nursing Certification / Registration / License Registered Nurse license issued by the state in which the team member practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). Required to have a valid driver’s license issued by the Division of Motor Vehicles and acceptable vehicle operator record Travel required within designated geographic area Work Experience Minimum of two-years of recent nursing experience in an acute bedside setting or emergency nursing Knowledge / Skills / Abilities Demonstrates progression to provide holistic patient care in the home and the ability to view clinical situations from a patient and family focus. Provides ongoing in-home evaluation of patient, team, and unit outcomes and is empowered to take appropriate action when changes are needed while providing care in the home.
