Participate in care plan meetings and assist with the development and updating of comprehensive resident care plans. Ensure resident assessments accurately reflect each resident's clinical condition, functional status, and care needs. Monitor MDS schedules and ensure timely completion and submission of required assessments. Identify documentation discrepancies and collaborate with the appropriate departments to resolve them. Monitor changes in resident condition and ensure appropriate documentation and assessments are completed. Maintain accurate and organized MDS-related records.
