Registered Nurse (RN) - Outpatient Program (Per-Diem, Days or Evenings)

Overview Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. In order to support our mission, Helio Health is seeking a Registered Nurse (RN) to work on a Per-Diem basis for our Outpatient Program . The available schedule for this role will be Monday - Friday's, 8:00am - 8:00pm. The RN can choose their days and hours with preference for those available to work 4:00pm - 7:30pm . The Registered Nurse (RN) will provide comprehensive treatment to those affected by substance use, mental health, and other behavioral health disorders. Registered Nurses at Helio Health have a unique opportunity to use their training to provide a comprehensive approach focusing on patients’ mental health and wellbeing. Less laborious work with patients with less tasks related to ADL’s. Primarily taking vitals, passings medications, and identifying and acute needs of mental health and SUD related issues. Responsibilities Work under the supervision of the Nurse Manager, Service Director and/or Program Director and follows medical orders for patient care. Communicate with supervisor, medical provider and/or Medical Director regarding patients' medical concerns. Monitor patients' medical status, signs and symptoms of withdrawal and reports to supervisor and/or medical director. Work with other medical staff across the agency and in the community for continuity of patient medical care. Communicate with clinical staff regarding patients' medical concerns. Work directly with patients' in providing medical services, interpreting medical information and provide recommendations or make referrals for medical services. Develop, recommend, and review medical policies and procedures for all programs within Syracuse Behavioral Healthcare. Advise the agency and ensure the provision of services for medical and health related issues. Provide direct patient care of medical examination, implementation of medical orders, treatment of medical conditions and communicable and infectious disease reporting in accordance with the law. Execute and review public health education and screening protocols with regards to tuberculosis, sexually transmitted diseases, hepatitis, and HIV prevention and harm reduction in accordance with Article 27-F of the Public Health law and under the Mental Hygiene law in coordination with the Medical Director/Nurse Manager. Implement and carry out a medically based group for patients on the above topics. Serves on special review and quality assurance committees as assigned. Provide and document medical consultation, assessment, examination, and treatment planning and review consistent with agency policy and requirements. Other related assignments and duties consistent with training and experience. Qualifications Must be a licensed Registered Nurse currently registered by the New York State Department. At least one year of experience, or comparable training in the treatment of substance abuse and/or substance dependency. Basic computer skills required. Critical Time Intervention Licensed Practical Nurse (LPN) (Onondaga County) In Community

Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Licensed Practical Nurse is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The LPN will work on a Full-Time basis providing all services in the community and using a hybrid work model when not in the community. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities. Pay Rate: $25.00-$26.92 per hour In addition to Helio Health’s comprehensive benefits package, all Full-Time LPNs are eligible for a Sign-On incentive up to $4,000 ! Responsibilities Carry a primary caseload providing CTI services. Assess basic health and medical needs within their scope of practice. Coordinate health care with community medical providers. Provide rehabilitation, education, and support services. Works to create relationships with key providers in the community. Other duties as assigned by supervisor. All client visits are expected to be completed in the community, meeting people where they are at. Qualifications Must be a licensed practical nurse currently registered by the New York State Education Department. Experience and/or training in the treatment of severe mental illness and substance use disorders preferred. Basic computer skills required. Must have a valid NYS driver's license.
