Monitor patients' health remotely by analyzing data transmitted from home monitoring devices Triage patient needs by phone or video, directing them to the appropriate level of care, and coordinating with other healthcare professionals for complex cases. Educate patients on their health conditions, preventive measures, and how to use remote monitoring technology. Maintain accurate and detailed electronic health records, documenting all patient interactions, histories, and test results. Ensure video conferencing and other telehealth equipment are functional and assist patients with using the technology.
