MAJOR AREAS OF RESPONSIBILITY Plan of Care Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family. Family Educator/Advocate Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs, provide appropriate support at the time of death, including disposition of medications, notification of funeral home, etc. according to agency policy. Policies Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Communication Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home. Operations Attend interdisciplinary team meetings for full team discussions, consultation and evaluation of patient/family needs.
