Job Description JOB SUMMARY Under the supervision of the Administrative Nurse Manager, the Sr. Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment. Additionally, they are required to learn and apply the Patient-Centered Medical Home Standards and Ten Building Blocks of Primary Care. They are responsible for providing nursing care to patients in the ambulatory setting. The Sr. LVN will work as an important member of the care team, working closely with the RN and the physicians to deliver all components of nursing care, individualized to meet the patient’s physiological and psychosocial developmental level and needs. The Sr. LVN will be responsible for chart abstraction. The Sr. LVN consistently projects a professional and courteous demeanor, and shall comply with the Medical Center House and Telephone Standards.
