The Clinical Manager is a dynamic and experienced licensed Registered Nurse, Physical Therapist, or Occupational Therapist, who serves as the cornerstone of clinical excellence within the agency. This individual is responsible for the comprehensive planning, coordination, and management of all multidisciplinary home health services, ensuring that patient care is not only compliant and effective but also delivered with compassion and integrity. Key Responsibilities 1. Clinical Oversight and Care Coordination: Directs and oversees the entire lifecycle of patient care, from admission to discharge, ensuring adherence to physician orders and established plans of care. Conducts meticulous reviews and provides final approval for all clinical documentation, including OASIS data sets, plans of care (485s), and visit notes, to ensure accuracy, completeness, and regulatory compliance. Leads interdisciplinary case conferences, fostering a collaborative environment where physicians, therapists, and nurses work together to create holistic and effective patient treatment plans. Functions as the primary clinical expert and resource for field staff, providing real-time guidance on complex patient cases, challenging clinical scenarios, and evidence-based best practices. Continuously monitors patient progress against established goals and outcomes, proactively ensuring that care plans are modified to reflect any changes in the patient's condition. 2. Staff Management and Development: Provides direct, hands-on supervision, mentorship, and performance management for a team of field clinicians, fostering a culture of accountability and professional growth. Qualifications Education: Bachelor of Science in Nursing (BSN) from an accredited institution is strongly preferred. Licensure: Must hold a current, unrestricted license as a Registered Nurse (RN) in the state of California. Experience: A minimum of three (3) to five (5) years of progressive clinical experience as a Registered Nurse. A minimum of two (2) years of direct experience in a Medicare-certified home health setting is required. Previous supervisory or management experience in a healthcare setting is strongly preferred. This position will m ainly be at the Parnassus and UCIC campuses for a diagnostic radiology nurse (MRI and CT ) with the ability to float to other campuses as needed. Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. The RN functions as a member of the interdisciplinary team caring for patients who are undergoing procedures or exams in the Department of Radiology and Biomedical Imaging at any Radiology site. The RN carries out established nursing procedures to prepare patients prior to Radiologic procedures, during the procedure, and following the procedure. The RN assists with the management of patient care during moderate sedation and anesthesia cases. Qualifications Required Qualifications 2-5 years of recent Radiology nursing experience The ability to wear lead apron when required. Moderate Sedation competency required. This position requires flexibility to orient and work at all UCSF Medical Center locations. Preferred Qualifications Certification in Radiology Imaging Nursing preferred 2 years experience in either IR, ICU, ED, PACU, or procedural area. Job Summary: We are seeking experienced Physician Assistants or Nurse Practitioners who have urgent care or emergency room experience, treating both adult and pediatric patients, to join our welcoming and diverse community of San Francisco providers. We are considering fulltime, part-time or per diem (PRN) schedules. Key Responsibilities: Treat both pediatric and adult patient populations (all ages, 6 months old and up) Perform minor procedures (sutures, casts, splints, etc.), and provide preliminary reads of EKGs and X-rays. Make diagnoses and prescribe treatments/medications. Collaborative work environment with the opportunity to work with a dedicated Scribe at our busier centers. Work in state-of-the-art centers are equipped with advanced technology, including an integrated EPIC EMR. Collaborate with on-site Rad Techs and Medical Assistants for support. Dedicated Urgent Care Specialty – exclusively practice urgent care medicine – No Primary Care, DOT or Occupational Medicine in our urgent care position .
