Job Summary Responsible for overseeing activities within the team in collaboration with Manager/Director. Provides care to medically complex patients in collaboration with the physician in a specialized acute care setting. Works autonomously and has authority, responsibility, and accountability for assessment, planning, implementation and evaluation of wound, ostomy, and continence care program throughout the inpatient care areas. Serves as a resource for wound care management, including identification, assessment, treatment, and prevention. Provides specialized consultation on wound, ostomy, and continence issues for patients, families, nursing and medical staff. Serves as a consultant in the management of patients with draining wounds, fistula and tubes. Serves as a consultant for the treatment of patients with altered skin integrity due to pressure necrosis, drainage or incontinence. Works with physicians and nursing staff to provide updated information regarding wound care. Conducts rounds on all patients with wound/pressure ulcers on a daily and as needed basis. Full Time and Part Time positions are available for all shifts. We offer competitive wages and benefits to the right candidate. Responsibilities: - Assist residents with activities of daily living to include: bathing, dressing, grooming and toileting. - Transfer residents to and from activities and meals according to their individual service plans. - Serve meals to residents in the dining room or their apartments. - Record and report changes in residents' eating habits to supervisor. - Promote quality services within company, state and federal regulations.
