The Nurse Practitioner (NP) provider is one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. The NP Provider may work in a nursing home, hospital or community environment. He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed. Job Description The Pain Management Center provides care to outpatients. The hours of operation for the Pain Management Center are Monday through Friday 7:30 AM until 4:00PM. The responsibility of the department is to ensure high quality nursing care to patients having Pain Management procedures according to American Society of Pain Management Nurses (ASPMN), American Heart Association, ASPAN, NJ State Department of Health, CDC, CMS, OSHA and TJC recommendations. The department provides peripheral nerve blocks, nerve injections, nerve ablations, tenotomy and epidural procedures. The scope of care includes independently assessing patient needs, making collaborative decisions relative to all phases of care, pre-procedure, intraprocedural, and recovery while maintaining patient safety, monitoring, and high-quality patient care. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information.
