Job Summary Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel. Essential Functions Consults with and keeps supervisor informed of unit activities, needs and problems related to patient care. Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift. Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner. Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assists in patient admission, transfer and discharge procedures. Assist with handling and care of patient belongings and other personal property. Answers patient call lights, telephone, and paging system and responds as appropriate. Able to release, remove and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist RN and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening mild container, cutting food for patient. Assists patients with range of motion exercises, and movement to wheelchair or activity areas. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Job Summary: Collaborates with Education Consultants, Managers, CNS staff and Physicians to develop, design, deliver and evaluate/improve education strategies and special projects/programs based on clients departmental goals and needs, as well as organizational goals/objectives. Implements education programs and strategies and coordinates to meet regulatory program requirements and organizational needs. Participates in competency assessments and evaluations of new and existing staff. Contributes to ongoing employee performance appraisals. Essential Responsibilities: Develops, coordinates and evaluates programs to meet regulatory requirements and organizational, departmental and discipline-specific needs. Designs, develops, coordinates and evaluates activities for continuing professional education for staff. Collaborates with consultants to develop special projects in professional education, development and learning. Communicates and markets education programs to a broad target audience using appropriate methods, and based on identified/assessed needs. Makes recommendations for cost/quality strategies, works within budget; serves as expert resource to staff and departments in defining and meeting educational, clinical, quality and cost outcomes and performance measures. Develops, implements, monitors and evaluates/improves the orientation, cross training and education/competency processes/systems for clinical staff and students. Evaluates performance issues and develops strategies for remediation. Responds to corporate and industry changes in clinical practice and professional education and provides necessary training, education and development which improve organizational performance. Participates in leadership projects, meetings, committees and improvement activities. Ensures compliance and documentation of organizational records based on regulatory agency standards.
