A LVN Charge Nurse (Licensed Vocational Nurse) is a licensed professional nurse directly under the supervision of the Assistant Director of Nursing Services. The Charge Nurse is responsible for staff assignment and provides overall supervision of resident care activities. LVN Charge Nurse (Licensed Vocational Nurse) QUALIFICATIONS • Graduate of an accredited school of nursing or an approved LVN program. • Current nursing licensure in the state in which practicing. • Strong organizational skills. • Possess, as a minimum, training in a skilled nursing care facility, hospital, or other related medical facility. • Possess leadership ability and the willingness to work with and supervise professional and non- professional personnel. • Knowledgeable of nursing and medical practices and procedures. • Ability to Read, write and speak the English language. • Possess mental and physical health. LVN Charge Nurse (Licensed Vocational Nurse) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Complete all required record keeping forms/charts upon the resident’s admission, discharge, transfer, etc. File in the resident’s chart and/or forward to the appropriate department. • Receive verbal orders from attending/alternate physician and transcribe the physician’s order sheet. • Initiate resident Plans of Care on admission. • Chart TPRs, blood pressures and blood glucose as required. • Order prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Review physician’s orders and transfer/preadmission information for completeness. • Prepare/administer medications as ordered by the physician. • Answer telephone inquiries as outlined in the facility’s established policies and procedures. • Chart licensed nurses’ notes in an informative and d3scriptive manner that reflects the care provided as well as the resident’s response to the care. • Forward new diet orders, and/or diet changes to the dietary department. • Transcribe physician’s orders to resident’s charts, Medication Administration Records, Treatment Administration Records and RNA sheets to assure that they remain current. • Verify the identification of the resident before administering medication or treatment. • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician. • Assure that narcotic records are current at all times • Admit, transfer and discharge resident. Assist in arranging transportation and packing as necessary. • Check resident charts for specific treatment, medication orders and schedules daily. Job Descriptions Manual Section3.6, Page1of7 Job Descriptions Manual • Administer professional services such as: catheterization, tube feeding, suction, applying and changing dressings/bandages, packs, colostomy and drainage bags, taking blood, sputum, and urine specimens, care of the dead/dying, etc., as established by the facility’s policies and procedures. • Make rounds with the physician and assist with examination and treatment. • Review medication sheet for completeness of information, accuracy in the transcription of physician orders and adherence to stop order policies. • Assist in carrying out restorative and rehabilitative programs to include self-help/care. • Perform routine charting as required. • Report all discrepancies of physician order, diet or charting to the RN Supervisor or Assistant Director of Nurses. • Check food in resident’s rooms that is brought in by the resident’s family or visitors in accordance with the facility’s established policies and procedures. • Initiate restraint assessment on all new admissions. • Monitor and document behavior of residents on psychoactive medications • Record Intake and Output, and complete monthly evaluation as per the facility’s policy and procedure. • Following completion of an approved competency check with an RN an LVN may reinsert a G-Tube on an established track only. SUPERVISION • Meet with unit personnel, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Review and evaluate assigned personnel and make written evaluations to the RN Supervisor or Assistant Director of Nurses. • Coordinate Nursing service with other departments and nursing units. • Notify the attending physician, RN Supervisor and Director of Nurses when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with the facility’s established policies and procedures. • Assure that a stock of medications, medical supplies, equipment, etc., is maintained at all times. Requisition equipment and supply needs as required. • Initiate investigation of accidents and unusual occurrences and make the necessary written report to the Director of Nurses as established in the facility’s policies and procedures. • Notify the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident has a significant change of condition or becomes critically ill or injured, as outlined within facility’s established policies. • Work with the facility’s consultants as necessary and implement recommended changes as directed by the RN Supervisor of the Director of Nurses. • Investigate complaints and grievances and make necessary oral/written reports to the RN Supervisor or Director of Nurses. • In cases of death, notify the funeral home and ensure that the body is properly prepared/cleaned for pick-up. • Evaluate emergencies and determine emergency measures to be used, including the use of chemical or mechanical restraints. Notify the RN Supervisor and physician and record and follow orders. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform such duties/services. • Assist in standardizing the methods in which work will be accomplished. Fill out and complete Accident/Incident Report forms on all such occurrences and chart such information in the resident’s medical records as outlined in the facility’s established policies and procedures • Assure that nursing personnel follow established nursing procedures. • Remind attending physician(s) to review treatment plans, record and sign physician’s orders, progress notes, etc., during physician visits. • Give/receive nursing reports upon reporting in and ending shift duty hours. • Check all residents daily to assure that prescribed treatment is being properly administered by nursing personnel/assistants and to evaluate their physical and emotional status. Record findings in the resident’s chart. • Assist in orientation of newly assigned personnel as well as supervise and instruct other nursing personnel. • Orient new residents/family members as necessary. • Assure that nursing personnel are providing adequate nursing car in accordance with established nursing service procedures. • Assure that all personnel follow established infection control and aseptic techniques when isolation procedures become necessary. • Report all hazardous conditions and equipment to the RN Supervisor and maintenance department immediately. • Keep the RN Supervisor informed about the status of residents, staff and related matters. • Assume the authority, responsibility and accountability of directing your unit/shift. • Follow required coaching and progressive disciplinary procedures. • Others as may become necessary and appropriate or as may be directed by the Director of Nurses or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected. 
A nursing assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
