Documentation: Charting is accurate, legible, dated, and timed. Documentation reflects nursing process and interventions and evaluations taken. Utilizes computer systems effectively and efficiently for optimal patient care. Clinical Outcomes: Discusses patient findings and progress toward outcomes with physicians and other members of the health care team. Job Overview The LVN Case Manager may provide daily care coordination, case management, coaching, consultation, and intervention to patients with one or more chronic diseases. Experience with physically or mentally impaired adults and/or the geriatric population. Three years of experience in public health nursing, acute care, case management, and/or in-home health care required, minimum of 2 years of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care preferred.
