Case Manager LVN will cover two facilities. $35-38/hr POSITION SUMMARY The purpose of your job position is to coordinate delivery of services to managed care and Medicare residents in accordance with current federal, state and local standards and regulations that govern the facility and in collaboration with the Administrator, Director of Marketing, and other facility staff. The Case Manager monitors and documents the cost effectiveness of treatment provided, coordinates the admission and discharge process, serves as the resident/family advocate, and acts as liaison to insurance and medical management professionals. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. Qualifications Two years experience as a Scrub Tech in multiple specialties or an equivalent combination of education and experience Able to deal with problems involving a few concrete variables in standardized situations General familiarity with medical terminology as it pertains to anatomy and surgical procedures for an operating room Who We Are At USPI, we create relationships that create better care.
