Essential Duties and Responsibilities for a Community Admissions Registered Nurse: An Admissions Registered Nurses presentation of hospice services to patients and families is individualized, sensitive to patient and family's fears, needs and goals, demonstrates a knowledge of guidelines for hospice eligibility and related HPCB programs, and appropriately identifies the level of hospice care. Demonstrates ability to perform assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings appropriately in the EMR. Performs pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions. Able to articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states. Able to articulate etiologies of symptom manifestation (nausea, vomiting, anorexia, fluid and electrolyte imbalance, diarrhea, constipation, mucositis, weakness/fatigue, dysphasia, edema, dyspnea, cognitive impairment, urinary retention, depression, delirium) related to a disease process. Initiates the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control. Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, diuretics. Organizes consistently to meet productivity standards, incorporates computer documentation into patient visit and completes documentation requirements within time parameters. Executes patient/family/caregiver teaching in all situations. Displays an ability to establish relationships and maintain/collaborate positive rapport with other access and hospice clinical staff and hospice support staff, hospital staff, and the medical community. Participates in the weekend, holiday schedule. Qualifications for a Community Admissions Registered Nurse: Education/Certification Graduate of an accredited school of nursing with current Registered Nurse licensure in NY Certification in Hospice and Palliative Care Nursing preferred. Support Aides will be employed for approximately 15-60 days before becoming eligible to be placed in our CNA training course. Support Aide Overview: You will begin employement as a Support Aide, learning the role of a CNA. Additionally, you will assist in other department as needed. Elderwood at Williamsville's CNA Training course will last for three weeks, Monday to Friday from 7:30a-3:30p (times may vary slightly). Until you are officially slotted for training, you will be designated on a specific shift as a Trainee. Once in the course - the first two weeks are classroom training; third week will be the clinical portion. Once you pass, your status (and pay) will be adjusted from Support Aide to CNA.
