Promote adherence to infection prevention and control practices, resident safety requirements, and established facility procedures. We are seeking a Registered Nurse (RN) for their Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The Registered Nurse (RN) provides comprehensive nursing services across the CRTP, ICM, and CTI. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Salary : $95,000 per year plus benefits. Schedule: Monday - Friday 9am-5pm Essential Responsibilities (including but are not limited to): Clinical Care -Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Engages in cluster feeding while seated. Monitors daily intake and keeps nurses informed. Records food intake on Nutrition Work Sheet. Making Beds Uses recommended amount of linen and diapers. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques. Cooperation Works well with co-workers and supervisors. Maintains appropriate relationship with residents, families and peers. Gives assistance to other departments: i.e. recreation, when necessary.
