Position Overview This caregiver will meet an immediate need to support an client in the afternoons. Role includes companionship, meal prep, light housekeeping, and taking the client on errands. This role focuses on ensuring a safe recovery environment, offering personal support, and helping clients regain independence while following their care plan. Caregiver must be willing to drive the client. Essential Functions Monitor and report changes in the client’s physical or mental condition to the supervisor. Support medication reminders (non-medical, per care plan). Prepare light meals and snacks according to dietary needs and assist with feeding if necessary. Help with light housekeeping, laundry, and organization to maintain a clean, safe environment. Provide companionship and reassurance. Accompany the senior on errands Document care provided and communicate with the care team to ensure continuity. Respect and maintain client dignity, privacy, and confidentiality at all times. Skills/Qualifications Experience with Alzheimer's and dementia care Strong observation skills and ability to recognize changes in condition. Knowledge of safe mobility and transfer techniques. Patience, compassion, and ability to provide emotional reassurance. Good communication skills to interact with clients, families, and the care team. Dependability and flexibility to meet varying recovery needs. Valid driver’s license and reliable transportation. Education/Experience Requirements CNA, HHA, or equivalent caregiving experience preferred. Current certifications in First Aid, CPR, and TB test (or willingness to obtain). Successful completion of background check and drug screening, with random checks thereafter. Physically able to perform duties, including safe lifting and mobility assistance. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. The NP/PA, whether the practice setting is the inpatient, ambulatory, or outpatient settings, provides optimal patient care, from diagnostic to prognostic matters. Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure. 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations. 5. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
