A licensed professional nurse under the supervision of a Registered Nurse who provides nursing care and services to residents in a long-term care setting. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Job Description Under the supervision of the UCSF Primary Care Administrative Nurse Manager, guided by policies and procedures of the Ambulatory Services, the Hospital Assistant II assists in the delivery of care to patients at UCSF Primary Care.The Hospital Assistant II will be primarily responsible for the members of their primary care teamlet as well as providing back up support to other teamlets. Job Description The Heart Transplant Nurse Coordinator for the Advanced Heart Failure Comprehensive Care Center provides direct care to patients with multiple, often complicated acute and chronic medical problems. The Nurse Coordinator is the primary point person for the program in the post heart transplant phase of care and works closely with the multidisciplinary team and multiple UCSF departments. The primary responsibilities (as assigned) of the Nurse Coordinator will be to serve as the primary coordinator for post-transplant services and facilitate and assist with all aspects of patient care following heart transplantation. Some key aspects of this position include providing ongoing patient support and education throughout post heart transplant care in both the inpatient and outpatient setting. This role will also require cross coverage for patients across the Advanced Heart Failure Comprehensive Care Center as needed. Qualifications Required Qualifications At least 3 (three) years of recent nursing employment experience Recent ambulatory RN experience in a cardiology clinic/practice OR RN coordinator experience in solid organ transplant clinic/practice Working knowledge of immunosuppressive therapies and management of end stage heart failure patients Experience with Nursing education/teaching classes to nurses or patients Prior experience working with Epic Strong MS Office computer skills (word, excel, power point), databases Excellent organizational skills, proficiency in meeting deadlines, ability to work independently, prioritize workload and manage numerous concurrent tasks in high stress environment Demonstrated excellent written and verbal communication skills Evidence of work on institutional projects, committees, and councils and/or processes at UCSF or other medical centers The flexibility to orient and work at all UCSF Medical Center locations in person on campus Demonstrated exemplary patient teaching skills and transferring of critical knowledge to patient and families with closed loop communication.
