Job Description UCSF Benioff Children's Hospital is recognized throughout the world as a leader in health care, known for innovation, technology and compassionate care. UCSF has one of the top children's hospitals in the nation, according to a ranking by U.S. News & World Report. Our 183-bed UCSF Benioff Children's Hospital has more than 150 specialists in more than 40 areas of medicine. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Nurse Practitioners on the PICU and TCUP Team provide care in rotating 12h shifts. Responsibilities Function % of time Duties Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 70 Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. The APP will be responsible for coordinating the care of patients in the hospital setting Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Job Description Interested in a dynamic, autonomous practice setting, working with a highly skilled and collaborative APP team? Consider joining our ambulatory care Cancer Center Infusion APP team. The Cancer Center Infusion APP team is responsible for overseeing the care of 50-75 patients per day (depending on size of unit). We cover five Infusion centers providing treatment for Oncology and non-Oncology patients. The Infusion APPs triage multiple issues in real time and ensure continuity of care through collaboration and communication with physicians/surgeons/APP colleagues as well as other clinical and administrative staff. Ultimately, we ensure timely patient care and treatment to meet goals of care, optimize the patient experience and ensure good patient outcomes. In addition to our infusion areas, we cover our three bed Cancer Center Acute Care (CACC) where we manage higher acuity Oncology patients to avoid unnecessary emergency room visits, hospital admissions and facilitate emergency services and admission when necessary. Duties of Hospice Registered Nurse Include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Conducts home visits and assesses/evaluates patient's status. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Attends and participates in Interdisciplinary Group Meetings. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Supervises ancillary personnel and delegates responsibilities when required. Qualifications: Graduate of accredited school of nursing Current RN license in California Current CPR Certificate Registered nurses shall have a minimum of (a) one year of experience as a professional nurse within the last three years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and current RN license. Minimum of two years experience, at least one of which is in the area of public, home care, or hospice nursing. Must have a valid CA driver license and an automobile that is insured in accordance with the requirements of state of California and is in good working order.
