Bowel and bladder assistance, maintenance rehab, proper understanding of least restrictive device regulations, proper utilization of restraint devices, special care needs. • Documents resident care accurately and legibly as per facility policy. • Notes resident changes of condition and reports same to LPN/RN. • Performs vital signs as directed. • Performs tasks and responsibilities of assigned shift. • Reports on duty, listens to and/or obtains report from Charge Nurse, receives assignment. • Makes two hour and PRN rounds on all residents. • Keeps unit neat and clean; straightens, nourishment station, clean and dirty utility room and other areas as assigned. • Performs procedures outlined for Certified Nursing Assistants in the procedure manual. • Assists in the psycho-social aspect of the resident by cross training in recreational therapy. • Ensure the resident is out of bed, dressed and ready to participate in activities of choice. • Assist with transportation, if needed, to and from activities. • Provide assistance with ADLs during activities if necessary. • Assist resident to participate in individual activities of choice, (ie: setup of equipment, positioning assistance, providing sufficient lighting and space). • Inform residents of various activities available to them even when Activities staff are not available. • Other duties and responsibilities as assigned.
