Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in St. Louis and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. What You’ll Do As a Certified Medication Technician, you will assist with medication administration and resident care under the direction and supervision of licensed nursing staff and in accordance with applicable regulations and facility policies. Your responsibilities may include: Administer medications safely and accurately within the scope of your certification and training. Follow physician orders, medication administration records, and facility procedures. Observe residents for changes in condition and promptly report concerns to licensed nursing staff. Provide compassionate, respectful, and resident-centered care. Communicate effectively with residents, families, and members of the healthcare team. Maintain accurate medication and care documentation. Follow infection prevention, safety, and regulatory requirements. Protect resident privacy and confidentiality. Assist with maintaining a clean, organized, and safe care environment. Work collaboratively with CNAs, nurses, and other team members. Perform other duties as assigned. What We’re Looking For Current Certified Medication Technician (CMT) certification recognized in Missouri. Ability to follow medication administration procedures and facility policies. Strong attention to detail and organizational skills. Dependable attendance and a professional attitude. Compassionate approach to resident care. Strong communication and teamwork skills.
