The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Other duties as assigned. REQUIRED Active and valid state-issued Certified Nursing Assistant (CNA) license Current CPR certification or the ability to obtain CPR certification. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross. Bachelor of Science in Nursing degree preferred. Current state license as a Registered Nurse required. Current CPR Certification required. One (1) year recent experience preferred. Acute care setting preferred. ACLS certification preferred in LTACH and Texas hospitals. Additional Qualifications/Skills: Knowledge of and adherence to current nursing theory and practice, infection control, and wound care standards of practice. Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS. Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients. Demonstrates critical thinking skills. Demonstrates general computer skills including: data entry, word processing, email, and record management. Effective organizational and time management skills. Effective written and verbal communication skills. Ability to maintain proper levels of confidentiality. Ability to work closely and professionally with others. Ability to maintain quality, safety, and/or infection control standards. Responsibilities The Registered Nurse provides professional nursing care to patients within assigned unit. The role is also responsible for patient assessments, planning, supervising, interventions, and evaluations designed to prevent complications of physical disability, restore optimal functioning, and help the patient adapt to an altered life style. Posted Total Compensation The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. It’s not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case.
