Overview Manages, coordinates, facilitates and supports the members of the interdisciplinary team in provision of hospice care to patient/families of VNS Health Hospice Care Program, including After Hours. Ensures the implementation of clinical/quality improvement initiatives for hospice services in compliance with regulatory requirements. Demonstrates knowledge and commitment to excellence in clinical practice and customer service. Works under general direction. *This role includes a $15K sign-on bonus, payable in two installments throughout the first year of work with VNS Health. Ensures that support and direction to patient’s families are provided in accordance with the patient’s Interdisciplinary Plan of Care/Hospice Care mission, objectives, policies and procedures. Participates in Hospice Team meetings, IDG management meetings, and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. Ensures full regulatory compliance with Hospice Conditions of Participation and standards through review, monitoring and audit (ad hoc / regular) of files, processes and procedures. Collaborates with Quality Improvement/Education in the development and implementation of quality improvement and educational activities to ensure professional practice standards are consistently met for each member of the team. Participates in the design, implementation, evaluation and modification of quality initiatives and educational plans. Initiates, coordinates, performs and assists in the analysis of clinical, regulatory and fiscal practice. Assists leadership in achieving Hospice Care goals, objectives, and fiscal targets by providing support with compliance and/or innovation to achieve improvement. Manages the day to day operations of the department, which includes effective caseload or visit management, achievement of productivity and revenue/expenditure targets for Hospice Care. Completes all work assignments, which includes audits, reports and projects. Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, and terminates staff and recommends salary actions as appropriate. Facilitates and manages the interdisciplinary group for provision of an effective and comprehensive hospice services to patient/families of VNSNY Hospice Program. Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a Registered Professional Nurse in New York State required Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred Education: Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field or the equivalent work experience required Work Experience: Minimum of three years clinical experience in hospice required Effective oral, written and interpersonal communication skills required Clinical management experience preferred Knowledge of personal computer operations, including MS Office applications preferred Pay Range USD $98,200.00 - USD $130,800.00 /Yr. Position Summary: We have an exciting opportunity to join our team as a Surgical Technologist-K5 OR-Evening Shift (37.5 Hours). In this role, the successful candidate Under the direction of a licensed professional nurse can scrub on all surgical procedures in the Operating Room and assist the physician in procedural areas. Job Responsibilities: Clinical Takes initiative to make recommendations to leadership for new and updates for Physician preference cards timely and accurately for a patient/physician. Utilizes patient procedure, position to determine the plan of care. Anticipates and gathers needed supplies, equipment prior to provision of care based on procedure preference card and escalates outstanding needs with the RN Circulator. Demonstrates knowledge of use, care and maintenance of instruments and equipment. Under the supervision of the circulating RN, assists on procedures as a secondary circulator. Reports unexpected events and outcomes to RN staff/leadership in a timely fashion, e.g., count, specimen, breaks in aseptic techniques within the surgical field. Provides On-call (where applicable) coverage proficiently. Care of Specimens In collaboration with the Registered nurse, ensures specimens are appropriately identified, labeled. Regulatory, Quality & Performance Improvement, Safety Supports the mission, philosophy, standards, goals and objectives of the institution, department, and service line. Contributes towards quality and performance improvement programs and unit/service specific metrics, data collection, and projects, e.g., turnover times, Ontime starts, Time Out, patient satisfaction, instrumentation, waste, sustainability, ACA/RCA, committee participation. Participates in programs/activities to promote satisfaction of patient, families, staff, and other disciplines. Incorporates new practices identified in patient care standards into plan of care. Responds to changes in patient care and standards for patient care activities. Demonstrates awareness of JCAHO, DOH, and CARF, AST standards. Communication Interacts with respect and in a positive manner. Collaborates with members of the interdisciplinary team and promotes collegial relationships. Gives a thorough report to RN Circulator, relief scrub person, leadership concerning count, equipment, instrumentation, or outcomes. Minimum Qualifications: To qualify you must have a High School Diploma or equivalent required. BLS Required (American Heart Association) Must be a graduate of an accredited Surgical Technologist program through NBSTSA. Certification required upon hire.. Required Licenses: Cert Surgical Technologist, Basic Life Support Cert Preferred Qualifications: One-Two year experience as a Certified Surgical Technologist in a large complex Operating Room.
