Nurse Practitioner/Allied Health Professional

The Advanced Practice Nurse (APN) provides medical assessment, diagnosis, and treatment of individuals experiencing substance-related withdrawal in a Withdrawal Management (Detox) setting. The APN functions under a collaborative agreement with a physician and is responsible for ensuring safe, evidence-based withdrawal management, medication management, and coordination of care in compliance with New Jersey Department of Health (DOH) and DMHAS regulations. Essential Functions Clinical Care & Withdrawal Management Perform comprehensive medical assessments upon admission and throughout withdrawal management. Diagnose and treat substance withdrawal syndromes, including but not limited to alcohol, opioids, benzodiazepines, and stimulants. Develop, implement, and adjust individualized withdrawal management plans, including medication protocols. Prescribe, administer, and monitor withdrawal-related medications (e.g., benzodiazepines, clonidine, buprenorphine, naltrexone, supportive medications) in accordance with scope of practice and collaborative agreement. Utilize standardized withdrawal tools (e.g., CIWA-Ar, COWS) and document clinical decision-making. Monitor for and respond to medical or psychiatric emergencies, including escalation to physician or transfer to higher level of care when indicated. Medication Management Conduct medication reconciliation on admission and discharge. Ensure safe prescribing practices, medication monitoring, and adherence to program protocols. Educate patients on medications, side effects, and treatment expectations. Collaboration & Care Coordination Collaborate with the Medical Director, nursing staff, counselors, and interdisciplinary team to ensure continuity of care. Participate in treatment team meetings, case reviews, and discharge planning. Coordinate referrals to ongoing SUD treatment, MAT programs, primary care, or higher levels of care. Documentation & Compliance Maintain timely, accurate, and complete clinical documentation in the electronic health record. Ensure documentation meets NJ DOH, DMHAS, Medicaid, and accreditation standards. Participate in audits, inspections, and quality-improvement initiatives. Adhere to all facility policies, infection-control standards, and patient-safety protocols. Education & Leadership Provide clinical guidance and education to nursing and clinical staff regarding withdrawal management. Support staff competency related to detox protocols, emergency procedures, and medication administration. Participate in policy development and protocol updates as needed. Job Summary Responsible for implementing and supervising nursing services in the medical detoxification and gives direction to unit assistants on shift. Assists in training and evaluation of medical staff. Implements nursing care in a medically supervised facility treating up to 16 individuals going through withdrawal from alcohol, opiates, benzodiazepines, and other drugs, in accordance with program procedures of the organization. Essential Functions Implements nursing care in accordance with program procedures established by the Medical Director and Nursing Supervisor and approved by the Program Administrator. Administer drugs, medications, treatments, inoculations, and tests as allowed by law. Implement verbal physician orders and authenticating in writing, within a reasonable time, and included in the patient medical record. Communicate with physicians, nurse practitioners, nurses, and pharmacists regarding patient care. Pre-screening of new patients for appropriateness for admission, including TB screening. Obtain physical assessment data and health history. Record and document into health record all observations of patient response, following CIW-Ar, COWS or other assessment tools utilized in program protocols Inventorying and caring for patients’ personal possession. Executes prescribed treatments and medical interventions, including transcribing and administering routine and prescribed medications in accordance with procedures established by the Medical Director and Supervising Nurse. Records patient observations and activities in patient records accurately and timely in accordance with nursing standards and organizational policy and procedure. May be designated Charge Nurse for a shift, with oversight of LPNs or nursing assistants. Screens and provides first response to pregnant in detoxification, including obtaining health history on mother, screening for TB and contagious disease. Maintains a safe and sanitary environment applying principles of medical asepsis and applying infection control techniques and universal precautions. Provides patient education on health care, as assigned or appropriate. Implements safety and emergency procedures during fire drills and other disaster procedures, with assistance to patients in the detox unit. Attends regularly scheduled detox staff meetings. Follows all policies and procedures regarding patient information protection, including alcohol drug patient confidentiality (42 CFR) and HIPAA (45 CFR). Home Care Registered Nurse Duties include: • Assess patient at start of care to develop a plan of care • Initiate Plan-of-Care and make necessary changes as patient's needs evolve • Evaluate pati ents in their homes • Instruct and advise patients and/or families as well as the home health aides in patients' care • Reassess patients every two months to determine any condition change as well as supervise the home health aide to ensure proper care Home Care Registered Nurse Requirements: • Valid & current New Jersey Registered Nurse License and Registration • Current Driver’s License • Means to drive • Ability to drive up to 80 miles if needed • Organized and Patient oriented • Able to work independently • Good oral and written communication skills • Minimum of 1-2 years clinical nursing experience • Licensed home care services (LHCSA) or Geriatric experience preferred • Hourly rate/By visit compensation • As assigned by DON or Administrator • Morning Hours Primarily, M-F • Spanish Speaking a plus • Road warrior Job Types: Part-time, Travel nursing, Per diem Pay: $25.00 - $85.00 per hour Medical Specialty: • Geriatrics • Home Health Experience: • Nursing: 3 years (Required) License/Certification: • BLS Certification (Preferred) • RN (Required) • CPR Certified Work Location: On the road
